HomeMy WebLinkAboutContract 39491-B5 CITY SECRETARY CONTRACT NO.��� APPENDIX B-5 In accordance with the Professional Services Agreement signed between Digital Intelligence Systems Corp ("Contractor") and the City of Fort Worth (Client) on October 12, 2009 City Secretary Contract 39491, "Professional Services Agreement," this Appendix B-5 is incorporated into the Professional Services Agreement and hereby reads as follows: 1. The term of this work order shall be extended to end when all services have been completed under this work order as contemplated herein, but not later than the end date set forth below. 2. This Work Order incorporates by reference all of the terms and conditions of the Professional Services Agreement, CSC No. 39491 specifically the services to be provided under Section 1 of said agreement. 3. The services to be performed under this Work Order are listed below: DATABASE ADMINISTRATOR Contractor shall provide Database Administrator services to the City. This position is anticipated to be filled as of the date indicated below and will remain in place working for the City of Fort Worth until completion of the contract (6 months. The estimated date of completion for the contract is 03/31/2011. A contract extension must be agreed to in writing and signed by both parties for any services provided by the Contractor past the anticipated completion date of 03/31/2011. Services to be provided by contractor follows: 1. Database Administration for over 400 databases across 40 different servers. This includes database monitoring, database tuning, security access, and all other types of administration for the SQL Server databases. 2. Report Writing using SSRS (SQL Service Reporting Services) as requested from any of the departments within the City of Fort Worth 3. Creation and deployment of SSIS packages on the SQL Servers. 4. Scheduling of database jobs on the SQL Servers. 5. During the duration, the database contractor may be included on several projects that are started throughout the City. The expectations of the contractor will be documented and given to the contractor (ex. time lines, project requirements, etc...).For each project, the business requirements will be gathered and a due date will be determined prior to the database contractor being given the assignment. 6. Upgrades and patches to the SQL Servers and databases. Database will be migrated from SQL 2000 to SQL 2005, and some from SQL 2005 to SQL 2008. Also, SQL Server patches will need to be scheduled and applied to the servers when released by Microsoft.
