WHEREAS, The City of Fort Worth and Hahnfeld Hoffer Stanford (Architect) made and entered into City
Secretary Contract No. 40243 (The Contract) which was authorized by the City Council by M&C C-24215,
the 11th day of May, 2010; and Amendment Nos. 1, 2, 3, 4 and 6 were administratively executed in the
amount of$22,350.00 for additional topographic surveys, cost estimates and underground utilities survey;
and Amendment No. 5 was approved by M&C C-24980 on June 14, 2011 to complete the design and
provide construction administration services in the additional amount of $985,732.50 plus up to
$63,680.00 in reimbursable expenses; and
WHEREAS, further amendment is necessary for Phase II additional design services to increase the scope
of work for additional topographic survey of approximately 2-acres; and
NOW THEREFORE, City and Architect, acting herein by and through their duly authorized
representatives, enter into the following agreement which amends the Contract:
1.
Article 1.1, SECTION I —SERVICES OF THE ARCHITECT, a paragraph is added to read as follows:
"Provide additional design services for Phase II services thus increasing the Civil Engineering
Consultant's proposed scope of work to include the additional topographic survey of
approximately 2-acres. The cost of this additional service is $4,500.00. The scope of services is
described more fully in the Hahnfeld Hoffer Stanford's letter dated September 27, 2011 subject:
Revised Proposal for Civil Survey Work, City of Fort Worth, Texas, Meacham Airport Terminal
Building Renovation — Phase 2 including attachments noted as Exhibits "A" and "B", which are
attached hereto and as Attachment"A" made a part of this Agreement."

Paragraph 1 of SECTION V-COMPENSATION TO ARCHITECT, is amended as follows:
"The total compensation for all of the assignments to be performed by Architect to complete the
Projects as described in CHARACTER AND EXTENT of ARCHITECT'S SERVICES as
addressed in City Secretary Contract No. 40243 and Amendment Nos. 1 through 7 and hereof
shall be $1,247,257.50, hereinafter referred to as the 'total fee', plus up to $76,880.00 in
reimbursable expenses."

All other provisions of this Contract which are not expressly amended herein shall remain in full force and
effect.

Meacham Airport Terminal Building Renovation-Phase 2
Dear Mr. Muruyama:
As discussed earlier I am sending you a revised proposal for the Civil Survey portion of the Phase 2
work. As you may remember during the proposal phase the limits of our site work were
undetermined. The Master Plan for Meacham Airport was not yet to the final selection phase and
there were multiple planning options for the Terminal Building site.
We originally proposed the landscape and Civil work on an assumed project site boundary of
approximately 6.6 acres during the contract phase. During Phase 1 the site survey boundary was
increased to supplement the Taxiway extension project, and again during Phase 2 to include the
Underground Utilities Survey (SUE).An area of approximately 2 acres has not been included in the
previous surveys and is the final area needed to complete the topographical survey of our entire
project site, (see attachment"Exhibit B").
The revised proposal from Teague Nall Perkins is attached as Exhibit"A".

Topographic Survey—Contract Amendment
Mr. Meza:
As requested by Eric Claycamp with Hahnfeld Hoffer Stanford Architects, this letter agreement serves
as an amendment to the Professional Services Agreement for Meacham Airport Terminal Improvements
to expand the limits of the topographic surveying for this project. Per the attached exhibit,the additional
topographic survey is approximately 2-acres and will be provided and included with the original survey
for the Terminal Expansion and will be provided under the terms of the original agreement. The
previous survey for approximately 14.5 acres has been completed and delivered to the architect and
owner. With this additional survey, the survey will be updated to make this an all inclusive survey
totally approximately 16.5-acres
Based on the project description the additional survey will be:
Expanded Scope of Surveying for Meacham Airport Terminal Addition:
Four Thousand Five Dollars($4.500)
This is for additional topographic survey only and no additional boundary,platting, easements, or S.U.E.
are included in the scope of this work. Once this amendment is signed and returned, the survey work
will begin within three working days.
