WHEREAS,, the City of Fort Worth (CITY) and Brown & Gay Engineers,
nc. (ENGINES. made and entered, into City Secretary Contract No. 38798,
(the CONTRACT) which was authorized on, the 16�h day of Jun 200.9 in the
amount of $ 384, 46,8 - and
WHEREAS the CONTRACT as aubsequently revised y.- Amendment Number
1 'in tie amount of $99, 068 which was authorized. on August 14, 2012,
WHEREAS, the CONTRAC T::: _nvol es engineering services for the
following prolect .-
2008 CIP Arterial Street R ber-Lso Road- an
WHEREAS, it has become necessary to execute Amendment No. '2 to the
CONTRACT to include an increased scope of work and revised fee
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the ollow.. n agreement,
which h amends the CONTRACT.-

Article l of the CONTRACT is amended to :include the additional
engineering services specified in a proposal dated Movember 11, 2013 a
copy of which is attached hereto and ncorpo ated. herein. The cost to
City for the ad itiona1 sign services to be performed by Engineer
totals $45, 120. (See Attached F'u .ding Brea Beset, Page -3-

Art cle -11 of the CONTRACT' is amended to provide for an increase i
the fee to be paid to Engi iee for all work and services performed. under
the Contract,, as amended, s -that the total fee paid by the City for all
work and services shall be an, amount of $528, 656.

Per our previous discussion, Brown & Gay Engineers, Eric. (BCE) proposes that the fol lowing additional
services be Budded to this contract:
L Water line lateral lead ad Ustincnt Detail.
2. Construction Phase Support Services
At the request of the City,, the following addit on�tl scope items have been identified to support
completion of the project and aid in schedule recovery, The additional work includes basic enginceri1ng
services for the creation of a water line lateral lead a(Ijustment detail, engineering analysis and response to
Requests for Infon ration and assistance in management of the contractor through completion of the
construction of the project as requested by the City froin the Notice to Proceed, rate(04/29/201-3)

Part 1. --""mater Line Detail
a. Design a water line lateral lead detail for construction of the proposed water line-tbr the project.
BGE will create a detail to adjust water line lateral leads to;yid the contractor in avoiding
conflicts with storm sewer or other utilities. ��'his detail will si
r gned and sealed by a
professional engineer licensed in the State of Texas.
Part 2--Construction phase Support
a. Response to Requests ests for Information

b. Constrijetion Contract Management Support
BGE will attend 6 construction coordination nieetings, make 3 project site visits as needed
and review 6 monthly schedules and pay requests as reqLiested.
In accordance with our contract, Article V, Section 'K of the STANDARD AGRIFEMENT FOR
ENGINEERING SERVICES, Brown & Gay Engineers, "h1c., requests,an equitable adjustment to our
contract to account for the additional. cost reqUired to perform the City's requested additional services.
The proposed man hour breakdown fort he additional services is attached. The proposed $45,120.00
hourly not to exceed tree for these services was detei-t-nined based on the time estimated to complete at the
hourly rates outlined in ExIiibit B-2, HOUrly Rate SchedUlefr coin,the s,rANDARD AGREEMENT FOR
LNG NGfNEERING, SERVICES. The MWBE participation for this amendment, is 0%,.
