JUSTIFICATION (REASONS) FOR CHANGE ORDER
IN ACCORDANCE WITH THIS CHANGE ORDER REQUEST, THE CONTRACTOR SHALL:Fumish all labor and material as per the proposed Change Order #1. This
change order includes additional pay items for the removal and reinstallation of the recently installed sanitary sewer line L-3032E on Race Street, from Station 1+00 to Station.
3+00 to allow for proper connection to an existing sanitary sewer service. The original plans indicated this service connection at Station 2+59; however, field verification
revealed the service was located at approximately Station 3+00. As a result, the main must be realigned and extended to capture the existing service.
In addition, this change order includes the substitution of five (5) planned 4-inch PVC sanitary sewer services with 4-inch ductile iron pipe (DIP) sewer services along line L-
3425 on W Beach Street. This modification is required because the existing storm line was found to be higher than shown on the design plans, resulting in insufficient cover
(less than 2 feet) over the proposed PVC service lines. The use of DIP will ensure adequate structural integrity and compliance with City standards.
Time: City staff supports the Contractor's request for an additional fifteen (15) calendar days to complete the added scope of work.
Cost: Staff has reviewed the contractor's proposed unit prices and compared them to pricing from recent similar projects and Unit 1 Water bid items. Based on this review, the
proposed costs are deemed fair and reasonable.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in
full (both time and money) for all costs arising out of, or incidental to, the above Change Order. The Project/Program Manager signing below acknowledges
that he/she is the person responsible for the monitoring and administration of this contract, including ensuring all performance and reporting reouirements.
