FIRST AMENDMENT TO
FORT WORTH CITY SECRETARY CONTRACT NO. 63570
This First Amendment to Fort Worth City Secretary Contract No. 63570 ("Agreement") is made
between the CITY OF FORT WORTH ("City"), a Texas home-rule municipal corporation
through its duly authorized Assistant City Manager, and GENERATION UPLIFT, a Texas non-
profit corporation, hereinafter referred to as ("Contractor"), acting by and through Lisa McDaniel,
Founder/Executive Director, its duly-authorized representative, each individually referred as a
"Party" and collectively as the "Parties".
WHEREAS, the City and Contractor entered into City Secretary Contract 63570 for a term
that began on April O1, 2025; and
WHEREAS, the Parties wish to amend the Agreement to extend the term period to end on
November 30, 2026 and the agreement end date to January 31, 2027, to process invoices associated
with valid expenses incurred during the project term period.
WHEREAS, the Parties wish to amend the Agreement to add the language for
disbursement and to add Exhibit H- Disbursement Schedule Form.
NOW THEREFORE, City and Contractor do hereby agree to the following:
I.
AMENDMENT TO AGREEMENT
The following terms are hereby amended to either replace any conflicting terms in the
Agreement or shall be added to the terms and shall be binding and enforceable as if they were
originally included therein.
Agreement Documents:
9. Exhibit H— Disbursement Schedule Form
Section 2.4 of the Agreement is hereby amended as follows:
2.4 Contractor is authorized to receive disbursement for up to 25% of allowable budgeted
expenses submitted by EXhibit "H" Disbursement Schedule Form, before Action Plan approval.
To access remaining contracted amounts, Contractor shall have an approved Exhibit "D" —
Action Plan by the first quarter report submission. The City may review and request changes
to E�ibit "D" in order to determine if the submitted document is likely to achieve the intent of
the contract. Upon approval, contractor will be able to submit reimbursement requests on the
remaining 75% of allowable budgeted expenses for the remainder of the contract period. In the
event the Contractor is unable to gain approval on Exhibit "D" by the stated deadline, the City
will terminate the contract and approve up to an additional 10% of allowable budgeted expenses
to cover remaining eXpenses incurred prior to the termination date.

Section 2.5 of the Agreement is hereby amended as follows:
2.5 No Revisions on Final Reports The last day for the City to receive RFRs and
supporting expense documentation from the Contractor for this agreement is December
15, 2026. Any discrepancies on the final RFR or support documentation will be adjusted
from the final reimbursement amount with CCPD's sole discretion.
Section 2.10 of the Agreement is hereby amended as follows:
2.10 Last Day to Approve Modi�ications The last day the City will approve a Request
for Budget Modification for this agreement is October 1, 2026.
Section 3 of the Agreement is hereby amended as follows:
3. Term
3.1 This Agreement shall commence on Apri101, 2025 (`Bffective Date"), and shall end on
January 31, 2027 ("End Date"). With the exception of an extension, as outlined in Section 3.2,
all of Contractor's expenditures and program services under this Agreement must be completed
during the project term period Apri101, 2025 — November 30, 2026. The City reserves the right
to withhold the final Request for Reimbursement until all required documents have been
provided by the Contractor to the City. All final reports for this agreement must be received by
December 15, 2026.
3.2 Request for Extension The City in its sole and eXclusive discretion may extend the
End Date of this Agreement in order for the Contractor to conclude all work and fulfill objectives
set forth in Exhibit A. The Contractor agrees that this is not an increase of funds. To request an
extension, Contractor must provide written justiiication for the request no later than September
15, 2026. The City reserves the right to decline any extension request that is not in the best
interest of the City.
3.3 Liquidation Period Contractor agrees that no additional costs can be incurred after
November 30, 2026. Contractor has until December 31, 2026 to liquidate accrued expenditures
that occurred during the project term period Apri101, 2025 — November 30, 2026. To receive
reimbursement, Contractor agrees to report liquidated expenses using the Exhibit C form and
provide backup documentation by December 15, 2026.
Section 4 of the Agreement is hereby amended as follows:
4. Action Plan
4.1 Approval of Action Plan Contractor agrees that an Exhibit "D" must be approved by
CCPD by the end of the first quarter, July 15, 2025. Prior to this date, the City may review and
request changes to Exhibit "D" in order to determine if the submitted document is likely to
achieve the intent of the contract. Contractor is authorized to receive disbursement for up to 25%
of allowable budgeted expenses submitted by RFR, before Action Plan approval and per CCPD's

sole discretion. Upon approval, contractor will be eligible to submit reimbursement requests on
the remaining 75% of allowable budgeted expenses for the remainder of the contract period. In
the event the Contractor is unable to gain approval of "Exhibit D" by the stated deadline, the City
will terminate the contract and approve up to an additional 10% of allowable budgeted
expenses for cover remaining expenses incurred prior to the termination date.
4.8 The final E�ibit `B" Action Plan Progress Report is due December 15, 2026
II.
ALL OTHER TERMS SHALL REMAIN THE SAME
All other provisions of Agreement which are not expressly amended herein shall remain in full
force and effect.
III.
ELECTRONIC SIGNATURE
This Amendment may be eXecuted in multiple counterparts, each of which shall be an
original and all of which shall constitute one and the same instrument. A facsimile copy or
computer image, such as a PDF or tiff image, or a signature, shall be treated as and shall have the same
effect as anoriginal.
