Change Order No. 05 (C05 includes six CMRs with a total cost of $131,g00. In additio to the extra work and associated
costs of this change order, �he Contractor has requested an extension of the substantia� and final completion dates.

As part of this Chan e Order No. 5, the City agrees to accept the CREDIT, a�rees to extend the Substantial Completion
�I afe to lune 13, 20�3, establishes an equipment warranty date of Septem er 18, 2024, and waives liquidated damages
or late delivery.
It is anticipated that a final payment request will be submitted following approval of this Change Order.

CMR-17 Stationary Gantry Crane Power Supply
Dear Farida:
Transmitted here within please find a copy of Crescent's June 20, 2023 contract modification
request (CMR) for providing 480V power supply to the stationary gantry crane provided under
PCM 7. The stationary gantry crane requested in PCM 7 included a 208V power selection. During
shop drawing review the manufacturer said 208V power is not available for this type crane.
Therefore CDM Smith requested Crescent use the existing 30A, 480V circuit breaker in existing
panelboard SHP-1 and field route 3#10, 1#10G, and 3/4" conduit from existing panelboard SHP-1 to
the gantry crane control panel. The cost associated with this work is $20,000 with no additional
days.
CDM Smith has reviewed the price proposal and is of the opinion that it is fair and reasonable. CDM
Smith recommends the City accept this cost and issue a Change Order in the amount of $20,000.
Please advise if the Water Department desires this CMR-17 be incorporated into a Change Order.

CMR-18 I&C Modifications at Grit Pumps
Dear Farida:
Transmitted here within please find a copy of Crescent's June 22, 2023 contract modification
request (CMR) for replacing the discharge pressure gauges and flow switches for the grit pumps;
add maintenance isolation ball valves at the grit pump seal water solenoid valves, and making
PLC/HMI modifications to add Headcell drain sequence.
Upon completing the start-up of grit pumps, the Flowserve (Wemco) field representative brought to
our attention that the seal water pressure gauges and flow switches were incorrect sizes and the
they recommended replacing them. Additionally, after reviewing the seal water solenoid valve
configuration for the HeadCell flushing water system, it was determined that even though the
flushing water system is used during start-up and shut-down of a grit unit, providing additional ball
valves at the solenoid valves would provide for better maintenance access of the solenoid valve
while keeping the flushing system in service. CDM Smith requested a cost proposal for the ball
valves and it is included herein.
Furthermore, after reviewing the HeadCell drain sequence, CDM Smith provided clarifications in
Field Order 004 for the drain sequence that requires modifications at the PLC and HMI. The cost
associated with this work is $25,000 with no additional days.
CDM Smith has reviewed the price proposal and is of the opinion that it is fair and reasonable. CDM
Smith requests that Crescent salvage and turn over the pressure gauges and flow switches to plant
staff. CDM Smith recommends the City accept this cost and issue a Change Order in the amount of
$25,000. Please advise if the Water Department desires this CMR-18 be incorporated into a Change
Order.

CMR-21 Additional Circuits for Heat Tracing
Dear Farida:
Transmitted here within please find a copy of Crescent's September 14, 2023 contract modification
request (CMR) for extending electrical conduits to feed the heat trace and thermostats to the wash
hydrants on top of the grit structure and to the grit pump room sump pump drain lines that are
routed on top of the grit structure. These two piping systems were not included in the heat tracing
electrical schedule.
CDM Smith has reviewed the price proposal and is of the opinion that it is fair and reasonable. CDM
Smith recommends the City accept this cost and issue a Change Order in the amount of $11,900.
Please advise if the Water Department desires this CMR-21 be incorporated into a Change Order.

CMR-24.1 Media Bed Sample Ports
Dear Farida:
Transmitted here within please find a copy of Crescent's Apri124, 2024 price proposal for installing
permanent sample ports for the Headworks Biofilter. The sample ports help facilitate balancing the
air flow between the biofilter beds for testing and balancing. During the testing and balancing call, it
was decided that these ports would be helpful to plant staff for future balancing activities so they
will be permanently installed for plant staff to use in the future. The cost associated for this work is
$4,000.
CDM Smith has reviewed the price proposal and is of the opinion that it is a fair and reasonable
price. CDM Smith recommends the City accept this cost and issue a Change Order in the amount of
$4,000. Please advise if the Water Department desires this CMR-24.1 be incorporated into a Change
Order.

CMR-25.1 Solenoid Valve and Strainer Changes
Dear Farida:
Transmitted here within please find a copy of Crescent's July 8, 2024 contract modification request
(CMR) and related RFIs 135, 136, and 137 for replacing the Magnetrol solenoid valves with ASCO
solenoid valves for the grit pump seal water, grit pump flushing, and headcell flushing systems. The
1/z" seal water solenoid valves need replaced with normally closed (NC), energized to open (EO) to
match the pump control logic. The 1-1/2" Magentrol valves are malfunctioning and causing
oveheating. Each size of valve was sent back to Magentrol and their report in RFI 137 suspected the
service water quality is gunking the piston and causing the valve to not close/open properly. Plant
staff indicated they have not had any issues with the ASCO valves and service water quality. RFI
135, 136, and 137 directed Crescent to replace all the Magnetrol solenoid valves with ASCO valves
and replace all the provided strainers with mesh sizes recommended by valve manufacturer.
Crescent is requesting 99 calendar days be added to the final completion date.
CDM Smith has reviewed the price proposal and time extension and is of the opinion that it is fair
and reasonable. CDM Smith recommends the City accept this cost and issue a Change Order in the
amount of $58,000 and extend the Final Completion date by 99 calendar days. Please advise if the
Water Department desires this CMR-25.1 be incorporated into a Change Order.
