JUSTIFICATION (REASONS) FOR CHANGE ORDER The proposed change order is needed for the contractor to address additional items regarding the tree mitigation required as part of this project, safety of the left turn lanes, as well as remove two Fort Worth Developer Signs. For mitigation of the tree removals in the medians along the 103312: S Hulen St & W Risinger Rd intersection improvements project, the plans and tree permit call for planting seven trees in Chisolm Trail Park. Due to current conditions in the park, and additional coordination with CFW Park and Recreation Dept, this change order will add additional bid items and quantities for the removal and mitigation of an additional tree, tying the eight trees in to the existing irrigation system, and upgrading the irrigation from a battery operated to a solar powered unit. With the updated alignment of the left turn lanes, additional guide signs and pavement markings are proposed to guide motorists in the left turn lanes. This change order includes efforts to remove and dispose of two Fort Worth Developer signs in the medians along 103312: S Hulen St & W Risinger Rd. Additional funding is not being requested as part of this change order because there is a construction allowance in the contract which will be able to cover the additional items needed. The contractor has honored the pricing of the existing contract for all of the current bid items. There are three new bid items proposed, which accounts for the additional effort required for the tree mitigation and sign removals. The City has reviewed the proposed unit costs and consider them reasonable. The five days requested are to provide time for the contractor to complete the additional work in this Change Order, including material procurement of the additional quantities.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in full (both time and money) for all costs arising out of, or incidental to, the above Change Order. The Project/Program Manager signing below acknowledges that he/she is the person responsible for the monitoring and administration of this contract, including ensuring all performance and reporting requirements.
