HomeMy WebLinkAbout063651-AD3 - General - Contract - 22nd Century Technologies, Inc.CSC No. 63651-AD3 Addendum 3 Statement of Work This Addendum 3 Statement of Work (SO� is made under the terms and conditions established in the MASTER PROFESSIONAL SERVICES AGREEMENT between the City of Fort Worth ("City") and 22nd Century Technologies, Inc. ("Vendor"), the same being Fort Worth City Secretary Contract (CSC) 63651(the "Master Agreement" or "MSA"). This Statement of Work is made for the following purposes, consistent with the services defined in the Master Agreement: Providing temparary staffing services to the City of Fort Worth for the positions listed below. In particular, Vendor shall perform those functions identified below in the Scope of Services, as such services are more fully detailed in the MSA. Section A. Scope of Services & Schedule Department Requesting Position(s): Transportation & Public Works/ Stormwater Management Division Assignment #1 Name of Individual(s) Filling Position (if known): Unknown Position Requested (Must be in Exhibit B of the MSA): ADMINISTRATIVE ASSISTANT # of above Positions Requested: One (1) Projected Start and End Dates for Assignment: Jan. 5, 2026 — August 28, 2026 Anticipated Total # of Hours: 1300 HRS Hourly Billing Rate (Must be in Exhibit B of the MSA): $ 23.40 hourly Total Anticipated Cost: $30,420.00 (SCOPE OF WORK FOR ANTICIPATED POSITION) Individual Tasks —Temp Personnel QA/QC of vendor produced document scans a. Perform quality checking for scanned documents received from vendor, which involves comparing original documents with scans/pdf, and noting errors and documents that need to be corrected. b. Coordinate with other staff and vendor on managing the transfer of documents and scans. 2. Drainage easement research and coordination a. Assess and review archive/historical plats and documents b. Scan/capture easement document c. Data entry of attributes required d. Enter update requests into database for GIS updates e. Perform additional easement research utilizing Tarrant County website and central library location 3. Provide back-up support in contacting customers/residents via email or phone to schedule site investigation meetings for the Stormwater Infrastructure Team 4. Scanning of archived Stormwater drainage and Floodplain documents, comparing to existing documents, other document management and archiving work as assigned. OFFICIAL RECORD CITY SECRETARY FT. WORTH, TX 5. Report monthly on progress made on scanned document QA/QC, easement data collection, and coordination efforts 6. Additional miscellaneous administrative tasks as needed. Section B. Compensation, Reimbursable Expenses, and Billing Terms: Processing and payment of fees, including the structure of payments and invoicing thereof, shall be in accordance with the Master Professional Services Agreement. The total fee that shall be paid by City to Vendor for these services shall be in accordance with the Master Service Agreement and, specifically, Exhibit "B"- Price Schedule. In submitting invoices, Vendar shall reference the appropriate Statement of Work. Further, the Vendor shall separately list the total amount for fees in which invoices were previously submitted during the then-current annual contract period (either initial term or renewal period) and the total amount of fees for the entire period, including those amounts being requested under the then-current invoice. Vendor shall submit invoices for Fees to Shweta Rao within Transportation &Public Works Department (Shweta.Rao(c�fortworthtexas.�ov) and also submit a copy to the Talent Acquisition Manager in the Human Resources Department, 100 Fort Worth Trail, Fort Worth, Texas 76102.
