Dear Mr. Boyer:
This letter is a request for supplemental authorization to provide for additional surveying services.
On 5/24/99 and 5/28/99; design review meetings were held for the project. At those meetings, the
., following design changes were requested that will require additional surveying and easement
preparation. ,
L-43 8
This existing line runs under a building just north of Magnolia Street (see attached drawing). At
the design review meeting, it was decided to abandon the portion of the line under the building, and
re-route L-438 approximately 275' east along Magnolia and tie into L-437.
Approximately 275' of additional survey is needed along Magnolia Street between L-438 and
L-437 to complete design plans for this change in alignment.
L-464
'I'he City sewer layout map indicates that the existing line north of Magnolia Street ends at Sta.
2+SOE, and design surveys were ended at that point. Field investigations indicate that the line
actually extends further north to serve residences in the north half of the block. Our current scope
of services does not include survey for this additional area.
Approximately 225' of additional survey and preparation of at least 2 additional easements
will be needed to complete design plans in this area.

This existing line is located in an easement on property primarily owned by Hams Methodist Fort
Worth/Texas Health Systems (see attached drawing). Our current scope of services calls for
replacementlrehabilitation of this line in its existing location. At the design review meeting, it was
decided to abandon the existing line and construct a new line in 5 Ave.
Apprbximately 780' of additional survey is needed along Sth Ave. to complete design plans
for this change in alignment.
The estimated total fee for this additional work is $4,774. A detailed breakdown and summary of
estimated fees are attached.

MEMORANDUM
TO: A. Douglas Rademaker, Director, DOE and
Mike Groomer, Assistant City Manager
FROM: Tony Sholola, DOE S.
DATE: August 23, 1999
RE: DESIGN FEE AMENDMENT NO. 1 TO MAIN C1C4B DRAINAGE
AREA SANITARY SEWER SYST M REHABILITATION AND
IMPROVEMENTS (GROUP 6, CO TRACT 2), PART 7, DOE NO.
2113, SEWER PROJECT NO. PS46- 70460410230
The proposed amendment will reimburse Turner Collier & Braden, Inc. (TCB) for addi-
tional design services on the project as requested by the City.
The Water Department requested that Lateral L-438 be rerouted from under an existing
building into public right-of-way. It is also necessary to replace a 250 ft. section of Lat-
eral L-464. Field investigations revealed the existence of this segment of the line. Two
permanent easements are required to replace this line. In addition, it is necessary to relo-
cate lateral L-440 currently located on Harris Methodist Hospital right-of-waX.
Turner Collier & Braden, Inc. has requested reimbursement in the amount of $4,774.00
for the additional design services they performed thereby increasing the contract fee from
$234,570.00 to $239,344.00. I believe this fee is fair and reasonable for the additional
services that were provided.
Please authorize this additional fee by your signatures on the attached contract amend-
ment form.
