WHEREAS,AS, Tho City of Fort "worth and Quorum Architects (Architect) made and entered into City
Secretary Contract o....44432 The Contract) which, was authorized y the City Council oy i &C C-262391
the 7th day of May, 20131- andr.
WHEREAS, further amendment is necessary to rcim blluurso the Architect as part of the design services
agreement.
NOW THEREFORE, City and Architect, acting heroin by and through their dully authorized
representatives, enter into the following agreement which amends the Contract;
1.
Article 1..1, SECTION I SERVICES of THE ARCHITECT, the following paragraphs are added to road
as follows-,
Provide additional design services for the design for the placement of temporary office facilities,for
Code Compliance, The cost of this additional serv,ice is $7,400.00, plus, $o.00 in reimbursable
expenses. The scope of services is described more fully in the Quorum Architects Proposal dated
January 20, 2014, subject, Proposal for Additional Services, North Service Center, phase 1,
Quorum Project fro. 12113, PO-11 3-000777131, CS44432 North Service Center which is attached
hereto as Attachment`A" and made a part of this Agreement.,,

Paragraph 1 of SECTION V-- COMPENSATION S TION T ARCHITECT, is amended as follows;
"The total compensation for all of the as i nmeniits to be performed by Architect as described in
CHARACTER AND EXTENT of ARCHITECT'S SERVICES S hereof shall be a fixed fee of

Dear Dalton:
We are pleased to respond to your request for an Additional Service proposal for Architectural services for the
North Service Center,for the City of Pt Worth. It Is our understanding that Code Compliance would like to set
up trallers near the site for eapproximateiy 1 '-15 officers to use as an office for o year or so until the service
center 1e constructed. In order to avoid Interfering with the construction activities,the City would lime to set the
trailers on the small triangular site Immediately east of the welll e1te,NE of the Hillslhire Drive and Shire Meadow
Drive Intersection. This proposal Is to provide necessary design services,to assist the City in developing that
site to a level required to accommodate the trailer. Fisted below Is our proposed scope for this project..
Architectural --Quorum,along with consultants,will provide Architectural,design to allow the City to develop
the parcel of land described alcove. We anticipate penerelly that our 'work will Include site survey and
development of utility drawings Including water and sanitary. We will also provide enough grading to Indicate
accessible parking and an access route In compliance with T"DLR requirements. cuorum will Include
preliminary and final layouts for approval by the city,Including trailer(s) location,,pavement fllex base)extent
and fencing. The trailer sizes wIll be provided to auorurn hued on the city's direction. We have included
attendance at a P O meeting.,
1. Civil and site Engineering — Quorum, through the use of _Tien Deotte Engineering Inc. will provide the
survey and civil enPlneerin services as described In attachment"'A"as pert of Hasid Services:.

Including,civil engineering,as listed below and detailed In attached back up documents. ire anticipated SBE

We anticipate direct expenses associated with printing and reproductions of Documents to he llnrritedl to$2000
and have therefore Included it in our Basic Services.
2. Late payments: Accounts unpaid 30 days after the invoice may be subject to a monthig service charge of
1.5076 or the legal rate)on the then unpaid balance.
SCHEDULE
1. In consideration of the city's desire to move into the trailers sometime this summer and therefore anticipate
completion of design within B weeks of a Notice to proceed assuming that we can have a.PDC(meeting after
the fourth week

ADDITIONAL INFORMATION
11. Quorum Architects, Inc. will provide the client with architectural services as required and agreed upon for
satisfactory and normal completion of this project. The architect shall exercise usual and customary
professional care in his efforts to comply with those laws,, cedes, ordinances, and regulations,which are in
effect as of the date of this agreement.
HAZARDOUS,MATERIALS
The Architect and their consultants shall have no responsibility for the discoverg, presence, handling, removal or
disposal of or exposure of persons to hazardous materials in any form at the Project site. The owner shail obtain a
hazardous mate-Hall evaluation,prior to beginning any demolition or construction on cite.

Thank you for your requelst for thi's proposal for civil engineering services for the
referenced r *ect. The project includes the Assign for tern rary buildings on a 36,000
square foot site. l have reviewed the project and offer the following proposal for your
consideration.

Survey of topographical features as identified. Will include locating the existing
structures, driveways, utilities and other planimetric featuires to include tree ties.
Obtain elevations and set a project bench mark.

A. Dimension Control Plea
Dimension control plan will include dimensions to all proposed improvements
by means of coordinates or dimensions. wJDEl will utilize site layouts provided by
the Architect to develop a geometrically saun l plan. Dimensions for parking,
and drive lanes, will be checked. Plan will also include horn izantal and vertical
control set by surveyor for use during construction.

B. W I ater and Wastewater Plan
JDEI will prepare a Water and Wastewater Plan for the construction of new
dornestic water and new domestic sanitary sewer. New service lines will be
shown from their source to within 5' of the proposed building footprint.
Connection points at the building footprint to be, provided to JDEL

C. Grading Plan
Construction plans for proposed on-site grading and paving. On-site grading
will be in compliance with current ADA, standards. Plans will, be created to
balance out and fill volumes and minimize construction costs. I

D. Coordination for the Relocation of Transformers, Site Electric, and Site Lighting
JDEI will coordinate with the Architect, the Mechanical/Electrical Engineer,
Lighting spelcia�li'sts and Utility Companies, for the reloc ation or installation of
electric and site lighting.

A. Drainage Plan and Analysis
Adhering to local storm water management policy, JDEll willdevelop, existing
and proposed site drainage plans. Thi's plan will show the existing and
proposed storm water drainage paths. JIDE1 assumes -that no detention design
w"ll be required for site developimen I I I
I t and all storm water will retain its existing
drainage patterns.

B. Erosion Control Plan
JDEI will develop an erosion control plan utilizing current best management
practices (BMP's). The erosion control plan will include, but not limited to, a
stabilized conis,tructi,on entrance, silt fence, inlet protection, a concrete
washout area, check dams, and a materials staging area.

Total Additional Services ree $ 10500.00
JDE1 will perform research, design, coordination, and calculations as needed for the
preparation of the survey and site/grading plan. JD0 anticipates the fees listed above
for the various elements, of the project. All costs, associated with the identified scope of
work, will be lump sum for the scope provided and will be billed as a percentage of
completed work as determined by JDEL
