WORTH, TX AMENDMENT No . 4 TO CITY SECRETARY CONTRACT No. 48211 CSC No. 48211-A4 WHEREAS, the City of Fort Worth (CITY) and MV Engineering, Inc., (ENGINEER) made and entered into City Secretary Contract No. 48211, (the CONTRACT) which was authorized by M&C C-27916 on the 13th day of September, 2016 in the amount of $ 263,877.00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment No.l in the amount of $19,725.00 which was administratively authorized on September 2, 2020, Amendment No. 2 in the amount of $62,256.00 administratively authorized on November 17, 2022, and Amendment No.3 in the amount of $123,187.00 which was authorized by M&C 23-0496 on June 13, 2023; and WHEREAS, the CONTRACT involves engineering services for the following project: Sanitary Sewer Rehabilitation, Contract 96, CPN CO2692; and WHEREAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in proposal attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $18,618.00. (Water: $14,387.00; Sewer: $4,231.00) 2. Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $487,663.00. 3. All other provisions of the Contract, which are not expressly amended herein, shall remain in full force and effect. A summary of the original contract amount, previously approved amendments, and the current amendment request is provided below for reference. Description Amount Original Contract Amount $263,877.00 Amendment No.1 $19,725.00 Amendment No. 2 $62,256.00 Amendment No. 3 $123,187.00 Amendment No. 4 (This Request) $18,618.00 Total Contract Amount to date $487,663.00 The requested Amendment No. 4 reflects additional scope associated with utility replacement coordination and design services necessary to advance the project to completion in accordance with current City standards. A detailed Level of Effort (LOE) spreadsheet is included with this amendment request. The LOE provides a breakdown of labor hours, classifications, and costs associated with the Utility Scope Revisions to support transparency in scope and fee allocation. Based on prior discussions and direction received, the following scope revisions are requested: 1. Cast Iron (CI) Water Lines - Include evaluation and replacement of all existing cast iron (CI) water lines located within the project limits. Vitrified Clay (VC) Sewer Lines - Include evaluation and replacement of all vitrified clay (VC) sanitary sewer lines within the project limits. 4. Rockdale Drive — Water Line Revision - Remove the previously proposed water line installation along Rockdale Drive. Based on verification with Field Operations, water service will instead be extended from the existing water line located on the west side. Exclusions The Amendment 4 scope is limited to evaluation and replacement of identified utility infrastructure (CI water lines, pre-1970 DI water lines, and VC sewer lines) within the project limits. The following items are specifically excluded from this scope unless otherwise authorized: subsurface utility engineering (SUE) investigations, right- of-way or easement acquisition, environmental assessments, private service line replacements beyond project Purpose of Amendment These revisions and additions are intended to: • Align the project with current City standards and asset management practices • Address aging and obsolete infrastructure within project limits • Ensure compliance with the latest design manual requirements • Facilitate development of complete, bid -ready construction documents Next Steps Upon approval of this amendment and prior toupdating the design, cost estimate, and project schedule accordingly, we will proceed with preparing exhibits to fully establish the limits of the improvements associated with the Utility Scope Modifications.
