AMENDMENT TO CITY SECRETARY
CONTRACT NO. 40873
WHEREAS, the City of Fort Worth ("City") and Fort Worth Housing Authority
("Contractor")made and entered into City Secretary Contract No. 40873 (the "Contract");
WHEREAS, the Contract provided funding to Contractor pursuant to a grant received by
City from the United States Department of Housing and Urban Development ("HUD");
WHEREAS, Contractor desires to be reimbursed based on the difference in fees received
from CDBG eligible households and fees received from unassisted households; and
WHEREAS, it is the mutual desire of City and Contractor to amend the Contract to meet
Contract objectives.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX
76102, acting by and through Susan Alanis, its duly authorized Assistant City Manager, and
Contractor, whose address is 1201 E. 13th Street, Fort Worth, TX, acting by and through Barbara
Holston, its duly authorized Executive Director, do hereby agree as follows:
1.
a. Exhibit A is deleted in its entirety and replaced with the attached Exhibit A —Revised.
b. This amendment is effective June 1, 2010.

AMAKA Child Care Center SCOPE OF SERVICES
PROGRAM SUMMARY
COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG)
June 1, 2010 - May 31, 2011 $27,000.00

SCOPE OF SERVICES
This Scope of Services is based on the proposal prepared and submitted by the Subrecipient through the City of Fort
Worth's annual Request for Proposal (RFP)process. The Subrecipient agrees to operate this Community Development
Block Grant(CDBG)program in a manner consistent with the program delivery stated in the approved proposal.
However, in the event of any conflict between the proposal and any provision contained, herein, this Agreement shall
control.
The Fort Worth Housing Authority contracts with the YMCA to operate the AMAKA Child Care Center. The AMAKA
Center will provide services to 30 infants and pre-school age children. All services will be provided at 1501 Stephenson,
Fort Worth, TX 76102 from 6:30 a.m. to 6:00 p.m., Monday - Friday, on school holidays and during vacations.
The purpose of the program will be to provide a mechanism for low income parents to become self-sufficient by
completing training or work experiences with the assurance that their children are being cared for in a safe, well
managed, positive environment. The specific objectives, goals and level of services to be provided are listed below along
with the geographical location of clients served. All services will be provided from June 1, 2010 to May 31, 2011.
The CDBG funds will be used to provide 225 days of partially subsidized full day child care service for 30 infants and
pre-school age children which is consistent with Exhibit C-Detailed Budget.

Based on the nature of the service provided, Subrecipient Fort Worth Housing Authority will maintain documentation
that verifies that 100%of clients served by the AMAKA Child Care Center program are income eligible with current
household incomes under 80%of Area Median Income(AMI)as established by the Department of Housing and Urban
Development (HUD).

Client Beneficiary Location (Neighborhood,District, Citywide...) Council District 8, Citywide
PROGRAM GOALS:
To provide affordable. quality child care services to 30 children and enable parents to improve the family
quality of life.
PROGRAM OBJECTIVES:
The primary objectives of this program is to provide licensed, affordable, quality child care services to enable
parents to work and/or attend school. The program curriculum is designed to enhance the cognitive
development and social skill development of low income pre-school children in the Butler Housing
Community and other areas in the City of Fort Worth. 90% of the children serviced for 12 months will achieve
90% of age appropriate skills.
PROGRAM SERVICES and ACTIVITIES Total Units of Services for Year
A. Provide child care service (units =one day per child) 6750
(30 children X 225 days) units of service per year.
*A unit of service is defined as one child receiving program service for one day.
**Total cost per day = $25.00 of which the City will pay the difference between fees
received by CDBG-eligible clients and fees paid by non CDBG-eligible clients.

See if this is better, instead of saying "within eligible line items" I've listed what line items increased and
decreased.
My problem isn't that the amendment is too detailed, its that the exhibit is too detailed, which leaves room for
everyone's needs to be met!

My problem with these is that it's so vague that no one would ever know what had been changed unless they took
out the original Exhibit C and laid it next to the new Exhibit C and compared numbers in each category (which is
ironic since your problem is that it's too detailed)
Is there some simple way to say in the Whereas clauses what we're doing, more specifically than how it's worded
now but without getting into dollars and cents?

Leann,
Here is a package that includes the revised budgets for AIDS Outreach and Samaritan House. The revised
exhibits were very difficult to generate because when people want budget revisions they only provide the
summaries and then it is very difficult to work that into the detail, so I added some language on the first detail
page of the exhibit re: " the amount charged is not to exceed the amount on the Program Budget Summary".
Having this much detail causes a lot of problems down the line. I think we need to simplify the budget exhibit on a
go-forward basis, but that is a discussion for another time.
As before, please let me know if you need any additional information or have questions on any of these. If they
are OK, please let me know so I can get them to the agencies for execution.

I have several agencies that are requesting fairly routine amendments to their contracts. I have all of them
prepared with the exception of the revised exhibits for Samaritan House and AIDS Outreach Center. With those
exceptions, I have attached for your review. Please let me know if you have any questions on any of them
