AMENDMENT TO CITY SECRETARY
CONTRACT NO. 40888
WHEREAS, the City of Fort Worth ("City")and Child Care Associates ("Contractor")
made and entered into City Secretary Contract No. 40888 (the "Contract");
WHEREAS, the Contract provided funding to Contractor pursuant to a grant received by
City from the United States Department of Housing and Urban Development ("HUD");
WHEREAS, Contractor desires to expand the scope of its contract by offering services to
CDBG eligible clientele at locations previously not included in Contractor's scope of work; and
WHEREAS, it is the mutual desire of City and Contractor to amend the Contract to meet
Contract objectives.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX
76102, acting by and through Susan Alanis, its duly authorized Assistant City Manager, and
Contractor, whose address is 3000 East Belknap, Fort Worth, TX, acting by and through John
Whitcamp, its duly authorized Executive Director, do hereby agree as follows:
I.
a. Exhibit A is deleted in its entirety and replaced with the attached Exhibit A —Revised.
b. This amendment is effective June 1, 2010.

Pre-School Child Care SCOPE OF SERVICES
PROGRAM SUMMARY
COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG)

SCOPE OF SERVICES
This Scope of Services is based on the proposal prepared and submitted by the Subrecipient through the City of Fort
Worth's annual Request for Proposal (RFP)process. The Subrecipient agrees to operate this Community Development
Block Grant(CDBG)program in a manner consistent with the program delivery stated in the approved proposal.
However, in the event of any conflict between the proposal and any provision contained, herein,this Agreement shall
control.
The Child Care Associates will provide Childcare services to 61 income-eligible Pre-School Children at Child Care
Associates. The Pre-School Child Care program will include the following tasks and activities; provide a full day of
preschool to children with structured learning activities, self-directed activities in small groups and individualized plans
for each child . All services will be provided at six site locations:North Fort Worth, 901 NW 30"Street, Fort Worth,
Texas 76106; Station 12,2410 Prospect, Fort Worth,Texas 76106; Southside Child Development Center, 959 East
Rosedale Street, Fort Worth, Texas 76104; Blanche,2900 Stalcup Road, Fort Worth, Texas 76105; Ellis,3620 Modlin
Street, Fort Worth, TX 76107; and Tom Rikard,3006 Schwartz Dr, Fort Worth, TX 76106 from 6:30 a.m.to 6:30 p.m.,
Monday—Friday.
The purpose of the program will be to provide childcare services to low-income children. The specific objectives,goals
and level of services to be provided are listed below along with the geographical location of clients served. All services
will be provided throughout the contract period.
The CDBG funds will be used to pay for subsidized childcare fees for qualifying parents which is consistent with Exhibit
C- Detailed Budget.
REGULATORY CLASSIFICATION:
National Objective Citation: 24 CFR 570.208(a)(2)(C) Limited Clientele
Regulatory Citation: 24 CFR 570.201(e) Public Service
Based on the nature of the service provided, Child Care Associates will maintain documentation that verifies that 100%
of clients served by the Pre-School Child Care program are income eligible with current household incomes under 80%
of Area Median Income(AM1)as established by the Department of Housing and Urban Development(HUD).

PROGRAM GOALS:
The goal is to prepare all children for their upcoming academic time in public school.
PROGRAM OBJECTIVES:
Families are encouraged to participate in all center activities, with a special emphasis placed on single parents,
teen parents and parenting fathers. Children who participate in our program are exposed to a variety of
activities daily. The centers are filled with many structured activities. Small group activities begin the day and
through out the day children participate in self directed activities in learning centers. Child Care Associates
provides childcare services with individualized plans and provides for children with special needs as
appropriate.

See if this is better, instead of saying "within eligible line items" I've listed what line items increased and
decreased.
My problem isn't that the amendment is too detailed, its that the exhibit is too detailed, which leaves room for
everyone's needs to be met!

My problem with these is that it's so vague that no one would ever know what had been changed unless they took
out the original Exhibit C and laid it next to the new Exhibit C and compared numbers in each category (which is
ironic. your problem is that it's too detailed)
Is there some simple way to say in the Whereas clauses what we're doing, more specifically than how it's worded
now but without getting into dollars and cents?

Leann,
Here is a package that includes the revised budgets for AIDS Outreach and Samaritan House. The revised
exhibits were very difficult to generate because when people want budget revisions they only provide the
summaries and then it is very difficult to work that into the detail, so I added some language on the first detail
page of the exhibit re " the amount charged is not to exceed the amount on the Program Budget Summary".
Having this much detail causes a lot of problems down the line. I think we need to simplify the budget exhibit on a
go-forward basis, but that is a discussion for another time.
As before, please let me know if you need any additional information or have questions on any of these. If they
are OK, please let me know so I can get them to the agencies for execution.

I have several agencies that are requesting fairly routine amendments to their contracts. I have all of them
prepared with the exception of the revised exhibits for Samaritan House and AIDS Outreach Center. With those
