JUSTIFICATION (REASONS) FOR CHANGE ORDER
This Change Order covers the following items: 1) Excavation for the construction of the headwall for Drainge Line A (project station 1132+63.51, LT 24.95') exposed two
previously buried and unknown concrete pipes that continue west under the adjacent commercial development for an unknown distance and terminate at the project station
mentioned previously. These pipes are presumed to be drainage pipes, based on visible water ponding around and in the pipes, and have a flow line elevation that is 18
inches below the flow line of Drainage Line A. In order to mitigate erosion concerns and allow for infiltration of water entering the project site from these pipes, the decision
was made to switch the rip rap material around Drainage Line A's headwall from 4 inches of poured concrete to 12 inches of stone. The City, project engineer, and contractor
are all in agreement that this is the best solution for this differing site condition. 2) During initial excavation for the drill shafts for the bridge support in the median of US 377
(project station 1077+03.62), an existing concrete substructure was discovered that is in conflict with the proposed locations of the drill shafts. In order to mitigate this conflict,
part of the substructure needs to be removed, the drill shaft locations adjusted, and an additional concrete cap constructed that spans between both drill shafts. The expanded
scope of work inside this median will require two additional weeks of traffic control. 3) In accordance with the City's Standard Specification 01 2975 and TxDOT's Standard
Specification Item 9, this Change Order will also provide PARD the ability to make Material on Hand (MOH) payment for the Handrail material to be incorporated into this
project and establish unit prices that will facilitate MOH payments.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in
full (both time and money) for all costs arising out of, or incidental to, the above Change Order. The Project/Program Manager signing below acknowledges
that he/she is the person responsible for the monitoring and administration of this contract, including ensuring all performance and reporting
requirements.
