AMENDMENT NO. 1 To, Parvathi Sreekumaran, P.E. 100 Fort Worth Trail, Fort Worth, TX -76102 We are submitting this proposal to request authorization for additional survey and redesign services associated with the sewer improvements on Parcel 40/41 (2109 S. Main St.). Since the original survey was completed, field conditions on the parcel have changed, resulting in a required shift of the previously designed sewer alignment. These changes directly affect the design basis and necessitate updated field data and revised engineering documents. Scope of Additional Services • Boundary Survey Surveyor shall perform a boundary reconstruction of the property located at 2209 S Main St, Fort Worth, TX. The approximate perimeter of new survey will be about 767 -ft. The linear footage of the alignment on this parcel is about 220 -ft. Refer to Exhibit 1 for limits of survey shown via red box. • Topographic Drafting — Newly collected survey data will be drafted and integrated into the project base files. • Easement Revisions — Two (2) easements previously prepared for Parcel 40 will be rewritten to reflect the updated alignment. • Sewer Alignment Redesign — The sewer alignment along Parcel 40 will be redesigned to accommodate the shifted conditions, including rerouting of the sewer along S. Main Street. Jacobs • Railroad Crossing Plan Updates — Because the realignment affects the railroad crossing, the corresponding plan sheets will also be updated. Summary of the effort towards additional services is shown in Exhibit 2. Distribution of the proposed fee between water and sewer is shown in Exhibit 3. Detailed level of effort is presented in Exhibit 4. Sincerely, Vibhuti Pandey, PhD, P.E.
