City of Fort Worth Change Order Request Project Name 2022 Bond Year 3 Contract 16 City Sec # 63830 Client Project #(s) 104317 Water, sewer, stormwater and paving improvements on various streets Contractorl McClendon Construction Company, Inc. This Change Order adds new bid items necessary to address unforeseen field conditions encountered during construction on Fox Drive and Grapewood Street. During construction on Fox Dr, it was discovered that two lots containing duplexes are served by separate private water services. To avoid addifional connections directly into the newly installed water main, the Water Department recommended installing bullhead meter assemblies and reconnecting the existing private services to the new assemblies. During street excavation on Grapewood St, the existing subgrade was found to consist of low -plasticity -index (PI) sandy material. Based on the actual field conditions, cement stabilization is more appropriate than the lime treatment included in the original contract. This Change Order adds the necessary cement stabilization bid items and reduces the quantities associated with lime treatment. The resulting savings from the reduced lime treatment quantities will be used to offset the additional costs within the paving unit. Cost: City staff reviewed the contractor's proposed unit prices for the new bid items and compared them with available average unit pricing for similar work. Based on this review, staff has determined that the proposed unit prices are fair and reasonable. It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in full (both time and money) for all costs arising out of, or incidental to, the above Change Order. The Project/Program Manager signing below acknowledges that he/she is the person responsible for the monitoring and administration of this contract, including ensuring all performance and reporting requirements.
