PCM Description Provide UV High Water Level System Shut Down PCM No. 003
The Owner proposes to make the following additions,modifications or deletions to the work
described in the Contract Documents.Authorization to proceed with these changes must be
approved by Field Order or Change Order in accordance with the Contract Documents.
The goal of this PCM is to provide communications which will enable a High Water Level Sensor in the
UV channel to effect an emergency shut down of the UV submersible pumps.In PCM2AW,the initial
connection between the UV PLC and PLC RFP was made.No additional conduit or cable is required.
The required elements to implement this functionality are:
• Utilize spare wire and conduit from the remote UO panel to the UV control panel.
• Utilize one spare digital input at the remote UO panel,and monitor the UV high level signal then
perform UV pump high level interlock per revised UV System Operations Narrative,Section J.
1 have reviewed the construction documents with the Contractor and the proposed Time and Material
estimates provided by the Contractor.The estimated costs appear reasonable for the proposed work.The
work has also been coordinated with plant staff as well as our design team.No additional contract time is

PCM Description UV Tank Sampling Taps PClvl No. 004AW
The Owner proposes to make the following additions,modifications or deletions to the work
described in the Contract Documents.Authorization to proceed with these changes must be
approved by Field Order or Change Order in accordance with the Contract Documents.
Provide two sample points for the W tank. Provide 1"taps on the influent and effluent boxes per Trojan's
instructions. Locate the influent box tap on the east side and the tap on the effluent box on the north end of
the structure as shown on the attached drawing.On the effluent tap,extend 1"sample pipe into the effluent
box 1 ft. For both sampling locations,install a 1"threaded nipple in the penetration per coordination with
Trojan's recommendation. Connect to the nipple an elbow and a vertical run of pipe extending downward
from the elbow as shown on the drawing for the influent sample tap. A 1"stainless steel ball valve shall
be attached at the end of the vertical run. Underneath each sample tap,install a 6"PVC catch basin and 2"
PVC drain line,like those already in use at the plant. For the effluent tap,the drain line shall slope to drain
off the slab into the crushed rock area adjacent to the slab on the north. On the influent side,run the drain
to the crushed rock area to the south of the slab. Contractor shall install drain lines such that impedance to
access and tripping hazards are minimized. Unless otherwise specified,all pipe and fittings shall be
Schedule 40 304 SS. Contractor shall perform this scope of work in compliance with 15015 and related
sections.
I have reviewed the construction documents with the Contractor and the proposed Time and Material
estimates provided by the Contractor.The estimated costs appear reasonable for the proposed work.The
work has also been coordinated with plant staff as well as our design team. No additional contract time is
requested by the Contractor for this amendment.

Contractor Modification Request No. 2: Modifications to UV Platform
Dear Dr. Goderya:
Alan Plummer Associates, Inc. (APAI) is recommending approval of CMR-002 for the above-
referenced project.
CMR-002 relates to the modifications to tho ultraviolet ((JV) metal platform. This CMR
incorporates three minor changes. During review of the platform shop drawing, APAI raised a
concern about the split level of the grating. It was deemed to provide a less than optimal
arrangement for plant staff to navigate around the top of the tank. therefore, the contractor was
asked to make the platform finished floor all the same elevation,matching the top of the UV tank
elevation. Furthermore, since the metal fabricator would be re-detailing connections as part of
the modification, APAI requested, with the Owner's consent, drat the platform stairs be
symmetrical (changing the west side to look like the east side). Lastly, the contractor was
directed to make the dimensions of the platform orating width conform to the crnitract drawings.
1-he contractor's cost estimate for this work is:$8,992.2.8 with 1 io additional time roquested. The
relevant documents are attached.

Contractor Modification Request No. 3: Stop Logs
Dear Dr. Goderya:
Alan Plummer Associates, Inc. (APAI) is recommending approval of CMR-003 for the above-
referenced project.
GMR-003 relates to the stop logs fabricated to control the flow of water during installation of the
ultraviolet (UV) pump station submersiblo pumps. This issue originated via RFI••18, when the
Contractor stated that they needed to purchase plates and stiffeners for shutdown. After
deliberation regarding what the Contractor's contractual obligations were to perform this task,and
in light of the discovery that the check valves did not close all the way,APAI and Archer Western
agreed that the cost of this CMR would he crrt in half from the original$6,866.6O cost proposal.
The contractor's cost estimate for this work is$3,433.30 with no<additional time requested. The
relevant documents are attached.

On Friday 6/17/11, contractor is scheduled to have a shutdown to install the submersible sewage pumps
at the UV Pump Station. Through communication with the plant, it appears that the original stop logs to
prevent flow from the chlorine contact basin have been misplaced. Contractor's originall plan was to use
two existing steel plates, however they are too thick to fit in the grooves at the pump station. "l"herefore,
two steel plates and stiffeners will need to be purchased for the shutdown. Contractor will incur material,
labor and welding cost and will need to be compensated. Please direct contractor on how to proceed.
Please expedite a response because of the shutdown scheduled for Friday.

1. In accordance with the Contract Documents; G-001-General Construction Note No. 36, the
Contractor is responsible to provide whatever means and equipment necessary to control water
(luring construction.
2. In accordance with AW's email correspondence, the estimated value of the stop log(s) is $7,000.00.
T'he FWWD is willing to compromise and compensate AW for 1/2 of the stop log(s) value if the stop
logs are given to the plant when the work is completed.

Contractor Modification Request No. 5: Isolation Valve on UV Effluent grain
Dear Dr. Goderya:
Alan Plummer Associates, Inc. (APAI) is recommending approval of CMR-005 for the above-
referenced project.
CMR-005 relates to isolating the flow of the UV effluent drain from the strain on the influent side.
The Contractor brought up this question in RFI-32. The reason for adding the 2-inch stainless
steel ball valve is to ensure that untreated How ou the influent side does not bypass disinfection
and blend with treated flow. the contractor's cost estimate for this work is $994.97 with no
additional time requested. The relevant documents are attached.

The UV System has a drain line for the Influent Box and the Effluent Box. Both drain lines tie into each
other and they are not isolated. The water from the Influent Box is not disinfected and it can travel to the
Effluent side and discharge out of the system. A valve may need to be added to isolate the two drain
lines. In the attached drawing, contractor shows a proposed solution. Please direct contractor on how
to proceed.
