Home My WebLink AboutContract 29007-A4 AMENDMENT NO. 4 STATE OF TEXAS 8 CITY SECRETARY CONTRACT NO. 00_1 - AL4 (No M&C Required) COUNTY OF TARRANT 8 WHEREAS, the City of Fort Worth (City) and TranSystems Corporation Consultants (Engineer) made and entered into City Secretary Contract No. 29007 , (the Contract) which was authorized by the City Council by M&C C-19678 on the 29th day of July 2003 ; and subsequently revised by three (3) previous amendment; and WHEREAS, the Contract involves engineering services for the following project: Replacement of South Shore Sanitary Sewer Main 253 WHEREAS, it has become necessary to execute Amendment No. four (4) to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated January 18, 2011 , a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $ 7,415. 00 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $ 1,026,891. 00 OFFI0141 RECOnD�; CI Y f CRETARY 3. Water Department City of Fort Worth 1000 Throckmorton Street Fort Worth, TX 76102-6311 RE: Amendment 4 of M-253 Replacement City Project No. 29007, DOE 4027 Dear Madelene, TranSystems (TSC) appreciates the opportunity to provide additional services with respect to the above referenced project. We have prepared this amendment request for services regarding engineering and permitting. 1. Hydraulic Report $3,063 The pipe laid for M-252 Part I construction contract was found to differ from the contract documents. The Water Department with the assistance of TSC reviewed the results. TSC assistance included: • Review trends of slope and capacity from as-builts data. • Simulate flow conditions and determine the magnitude of a surcharging and characteristics of hydraulic jump. • Provide a brief letter report explaining the finding • Attend meetings(3) 2. Parks and Community Services Department(PACSD)Application $4,352 During the design phase, Mallard Cove Park was developed along the proposed sewer alignment. TSC assistance for a permit application and coordination included the following: • Standard application form, review of park plans, aerial exhibit of site with local parcels, misc exhibits, existing features and utilities. • Specification and plan coordination • Meetings (11) i. TSC Site Visits(2)—Application review, re-staking and tree locations ii. Site Visits with PACSD Project Manager and Water Dept(2) iii. Site Visit with PACSD Manager and Water Dept iv. Site Visit with Field Operations(only) v. PACSD Advisory Board Public Meeting at Haws Athletic Center vi. Meetings at PACSD with Water Dept.
