(ENGINEER) made and entered into City Secretary Contract No. 41739, (CONTRACT) which was authorized by M&C C-24821 on the 5t' day of April, 2011 in the amount of $240,000 . 00; and WHEREAS, the CONTRACT was subsequently revised by Amendment Number 1 in the amount $6,700 . 00 which was administratively authorized on May 29, 2012; and WHEREAS, the CONTRACT involves engineering services for the following project: Water and Sanitary Sewer Replacement Contract 2009, WSM-L; and WHEREAS, it has become necessary to execute Amendment No. 2 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated July 10, 2014, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $28,875.00 . 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid M M by the City for all work and services shall be an amount of M $275,575 .00 . City of Fort Worth Water Department 311 W Tenth Street Fort Worth, Texas 76102 Re: Fee Proposal for Contract Amendment No. 2 — Water and Sanitary Sewer Replacement Contract 2009, WSM-L City of Fort Worth Project No. 01478 (TNP Project No. FTW 10255) Dear Ms. Schwinger: Teague Nall and Perkins, Inc. is submitting this fee Amendment for the above referenced contract based on additional services requested. The City has requested that TNP provide the survey and design of 750 linear feet of 8" water line and 400 linear feet of 8" sanitary sewer design along Trueland Dr. from Rodeo St. to Gardenia Dr. to replace an existing undersized 4"water line and aging 6" sewer line. The City has also requested that TNP provide additional design services to eliminate an existing dead end water line located in Sarah Jane Lane,240'east of Ray Alvin Drive by extending the proposed water line 440' to an existing line in Rodeo Street. The following is a brief description and cost for the additional services to be provided on this project: 1. Design of Trueland Drive 8" Water Line and 8" Sewer Line The scope of services necessary to accomplish the design of the Trueland Drive water and sewer lines is addressed below. A. Design 1. Water line design effort will involve the proposed water line horizontal alignment, evaluation of fire hydrant coverage, locations and sizes of all water service lines and meter boxes,gate valve locations, isolation valve determination, and creation of plan sheets. It is assumed there will be one preliminary submittal and one set of City comments prior to these sheets being added to the WSM-L plan set. 2. Sewer line design effort will involve the proposed sewer line horizontal alignment, vertical profile, locations of all sewer service lines, verification that all TCEQ requirements are met, and creation of plan sheets. It is assumed there will be one preliminary submittal and one set of City comments prior to these sheets being added to the WSM-L plan set. B. Survey 1. An additional approximately 800 linear feet of topographic and design survey will be required within Trueland Drive right of way and the Trueland Drive/Gardenia Drive intersection to design the Trueland Drive water line from Rodeo St. to Gardenia Dr. The following surveying services will be provided: i. Research parcels/lots along proposed route, prepare a deed sketch, and provide addresses of adjacent parcels/lots. Provide a topographic survey including all improvements (water valves, manholes,curbs, etc.)and measure downs of all utility manholes from right of way to right of way for Trueland Drive. II. Design of Sarah Jane Lane 8"Water Line Extension Located 240' East of Ray Alvin St. The scope of services necessary to accomplish the design of the Sarah Jane Lane water line extension is addressed below. A. Design 1. Design effort will involve the proposed waterline horizontal alignment, evaluation of fire hydrant coverage, locations and sizes of all water service lines and meter boxes, gate valve locations, isolation valve determination, and creation of plan sheets. It is assumed there will be one preliminary submittal and one set of City comments prior to these sheets being added to the WSM-L plan set. B. Survey 1. No additional survey is required for the extension of the Sarah Jane Lane 8"water line as the proposed alignment is located within an area included in the original survey. Fee Proposal for Contract Amendment No. 2: $ 28,875.00 TNP proposes a fixed fee of$28,875.00 to accomplish the scope of services described above. This fee shall be full compensation for work required of TNP. Summary of Fees: I. Design of Trueland Water Line A-2. Water Line Design $ 10,250 A-1. Sewer Line Design $ 7,200 B. Survey $ 5,400 Il. Survey $ 0 Total $ 28,875 M/WBE Utilization: Due to time constraints, the City has directed TNP to use its own forces to complete these additional services more quickly.With this amendment,the overall M/WBE commitment will now be 20.1%, dropping below the City's M/WBE goal of 22.0%.
