HomeMy WebLinkAboutContract 29011 P,i : 39 !PJ AMENDMENT NO. 4 STATE OF TEXAS S CITY SECRETARY CONTRACT NO.c7'-, ( f (No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and Halff Associates, Inc. , (Engineer) made and entered into City Secretary Contract No. 24591, (the Contract) which was authorized by the City Council by M&C C-17215 on the 12th day of January 1999, and subsequently revised by three previous amendment; and WHEREAS, the Contract involves engineering services for the fol- lowing project: Water and Sanitary Sewer Main Relocations for FM 1187 Highway and BU 287P Highway Reconstruction. WHEREAS, it has become necessary to execute Amendment No. 4 to said Contract to include an increased scope of work and revised maxi- mum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agree- ment which amends the Contract: 1 . Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated July 28, 2003, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by En- gineer total $3,000 .00. 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid -to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $215,130. 00. 29 ORIGINAL � , 6 , 3. City of Fort Worth Department of Engineering 1000 Throckmorton Fort Worth,Texas 76102 Re: Amendment No. 4-Water and Sanitary Sewer Relocations for FM 1187 and BU 287P Highway Reconstruction, Section 2—BU 287P DOE No. 3700 Dear Mr. Sholola: Halff Associates is pleased to present this amendment for additional design services for the extension of Sanitary Sewer Line "A" north on Hartman Street off L-8892 along the north side of BU 287P (Mansfield Highway). The proposed sanitary sewer design revisions will allow the True Love Baptist Church to abandon their existing septic system and connect to L-8892 via Line"A". Amendment No.4 shall include the following additional services: • Conduct field surveys for portion of Hartman Street/True Love Baptist Church buildings not included in original design. • Revise construction plans to extend sanitary sewer Line "A" approximately 200' (Revise Sheet 10, add Sheet 10B). NOTE—Amendment No. 4 does not include changes to"up front" sheets (traffic control, erosion control, overall layout, etc.). • Coordinate design revisions with City of Fort Worth and TxDOT. A summary of the proposed budget for this amendment is as follows: Design/Construction Surveys(Gorrondona &Associates Inc.) $ 800 Design/Plan Preparation(Hal $ 2,200 TOTAL PROPOSED AMENDMENT NO.4 $3,000 The proposed M/WBE participation for Amendment No. 4 is 26.7% of the proposed fee. Ott, Assistant City Manager FROM: Tony Sholola, DOE DATE: August 7, 2003 RE: AMENDMENT NO. 4 TO WATER AND SANITARY SEWER MAIN RELOCATIONS FOR FM 1187 HIGHWAY AND BU 287P HIGHWAY RECONSTRUCITON. DOE NO. 3700, SEWER PROJECT NO. PS46-070460134200 This amendment will allow Halff Associates, Inc. to provide additional, engineering services on the contract due to the following: In order to provide public sewer service to a church located in close proximity to this project, it is nec- essary to install a sewer lateral 200 feet along the north side of BU 287P (Mansfield Highway) and connect the line to a project sewer lateral (L-8892). The church will connect to the new sewer lateral and abandon its private septic system once the public sewer lateral has been installed. Halff Associates, Inc., has submitted a proposal in the amount of$3,000 for the additional design ser- vices to be provided thereby increasing the contract fee from $212,130.00 to $215,130.00.
