Overview
Leaders greatly impact employee retention, productivity, profitability and
customer satisfaction. A 3601 leadership survey provides feedback from direct
reports, peers and managers and other key stakeholders, allowing leaders to
acquire a true picture of how others perceive their performance. 3601 survey
feedback can serve as a valuable learning tool in personal development
planning, enabling leaders to improve skills and relationships essential to their
company's success. Individual feedback surveys can also provide a baseline of
information for future reassessments so comparative data can be studied to
understand personal and professional growth.
Trade Secrets offers a 3601 feedback process that is highly customizable and
guarantees 100% confidentiality for each survey respondent. Our process not
only produces numeric ratings to survey questions but also provides respondents
the opportunity to give narrative comments. The survey instrument also allows for
open-ended questions. Consequently, report recipients typically benefit from
open and honest feedback from a high percentage of individuals invited to
participate in a feedback process that is tailored to the specific needs of the
recipient and his/her organization.
Assessment Requirements
For effective 3601 feedback, care must be taken to clearly define all rater
relationships. Richard Zavala will provide Trade Secrets with respondent email
addresses and their relationship to him (e.g., direct report, peer, manager).
Proposed Scope of Work
Trade Secrets will provide the following scope of work components:
• Custom survey design
o Approximately 40 respondents and no more than 30 survey
items
• Set-up programming
• Project announcement and invitation-to-participate
• Email-based distribution
• Data collection .,

Payment for Services
The total professional fee for this project is $1,126.48. A fifty percent down
payment ($563.24) is required to initiate the project. The findings of the
Executive Leadership Survey will be released upon payment of the
remaining balance of $563.24.
Any services performed beyond the stated Scope of Work will be billed at
$250.00 per hour, payable in advance. Paul Sweitzer or an authorized
representative of the City of Fort Worth must approve any such services in
advance.
Payments can be made by check or via American Express, Visa, or
MasterCard. All credit cards payments are deemed final.
Acceptance
Paul Sweitzer, or the City of Fort Worth, may accept this proposal by
returning one signed copy of this Executive Leadership Survey Proposal
with down payment to:

Project Proposal Expiration Date
If not accepted by January 1, 2006, the terms and conditions contained in
this tender of services may be subject to revision.
