WHEREAS, the City of Fort Worth (CITY) and National Switchgear
Systems North Texas, dba National Field Services, (ENGINEER) made and
entered into City Secretary Contract No. 45001, (the CONTRACT) which was
authorized by M&C C-26477 on the 24 day of September, 2013 in the amount
of $ 261, 510. 00; and
WHEREAS, the CONTRACT involves engineering services for the
following project :
Village Creek Water Reclamation Facilities (VC WRF) Power Systems
Arc Flash Analysis; and
WHEREAS, it has become necessary to execute Amendment No. 1 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated September 09, 2014, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $45,041. 00. (See Attached Funding Breakdown Sheet, Page -3-)

Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $306,551. 00.

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Per our conversation today I am requesting the additional funds to the amount of$45,041.00 be added
to our contract for the hours we incurred to collect information and add it to the SKM model for
additional electrical equipment not shown on the one-line. I have reduced the price from our Senior
Engineering rate of$186/hour to our Junior Engineering rate of$146/hour. In the attached Professional
Service Agreement on page 9 we called out the assumption that each 480V substation has on average 10
circuit breakers. We based our assumption on conversations with the plant electrical staff and other
engineers who had previously done arc flash studies at the plant. Each substation on average had 30
circuit breakers almost 3 times the amount we assumed. Ryan (NFS engineer) had to spend more time
onsite to collect the additional information and more time in the office entering in the data into SKM.
Please provide authorization for the additional funds and provide a revised contract for our records.
