COUNTY OF TARRANT
WHEREAS, the City of Fort Worth (City) and Ned K. Burleson &
Associates, Inc. , (Engineer) made and entered into City Secretary
Contract No. 27585, (the Contract) which was authorized by the City
Council by H&C C-19901 on the 19th day of March 2002, and
subsequently revised by two previous amendments; and
WHEREAS, the Contract involves engineering services for the
following project:
Water and Sanitary Sewer Replacement Contract 2001 WSH-N
WHEREAS, it has become necessary to execute Amendment No. 3 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:
1.
Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated June 14,
2004, a copy of which is attached hereto and incorporated herein.
The cost to City for the additional design services to be performed
by Engineer total $18,529.06.
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $135,447.87.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr. Conlon:
In response to your request, Ned K. Burleson and Associates is please to present this second
Amendment for additional services for design and survey of additional sanitary sewer lines and private
sanitary sewer re-routes to the existing contract. The amendment shall include the following additional
services:
• Design/Plan Preparation for an additional sewer line for continuation of relocation of M-146
into West 0 Street and Dorothy Lane. Survey scope includes detailed information within the
street right-of-ways and Lot 14, Block 13 of the Monticello Addition. Total linear feet of new
sewer line for this scope is 424 feet of 10-inch pipe. Survey work for this portion of the project
is detailed in Bullet'C'on the attached document.
• Design/Plan Preparation for new sewer line within Dorothy Lane to tie into relocated M-146
and connect to L-2129 in preparation of continued sewer replacement projects to the east of
the existing project area. Survey scope includes detailed information of the street right-of-way
for Dorothy Lane. Total linear feet of new 8-inch sewer pipe for this portion is 145 feet.
Survey work for this portion of the project is detailed in Bullet'C' on the attached document.
• Design/Plan Preparation for 10-inch and 6-inch sanitary sewer lines by pipe bursting and
insitu-form respectively for existing alignments for M-146 and L-2129 within Block 9 of the
Country Club Heights Addition. 416 linear feet of 10-inch pipe is to be replaced by pipe
bursting. 90 linear feet of 6-inch pipe for L-2129 will be replaced by insitu-form method-due to
an existing garage apartment that is located above the existing line.
Scope of survey for this portion includes detailed information for the small overgrown alleyway
and interior portions of Lots 3-7 of Block 9 of the Country Club Heights Addition. Detailed
survey for all structures, pavement,trees is vital for reconnection locations and temporary
construction easements. Survey work for this portion of the project is detailed in Bullet'B' on
the attached document.
• Design/Plan Preparation for sanitary sewer replacement and relocation of L-1101 within the
alleyway of Block 1 of the McCarts Addition. A total of 532 linear feet of 8-inch sanitary sewer
pipe will be installed for this portion. A detailed survey is to be performed within the alleyway
to a 20-foot extent'frorn the centerline of the existing sewer. Survey work for this portion of the
project is detailed in Bullet'N on the attached document.

• Detailed survey information is required for Lots 6-10, Block 12 of the Monticello Addition and Lots 1
and 15, Block 18 of the Country Club Heights addition for private sanitary sewer re-routes. These
seven lots have difficult obstructions and features that require a detailed survey of each lot to
complete the best design path for each individual sanitary sewer re-route. Survey scope for Block 12
is detailed in Bullet'D', and the survey scope for Block 18 is detailed in Bullet'E'on the attached
document. An estimated 700 linear feet of private 4-inch sanitary sewer pipe will be required to re-
route these lots to the proposed sewer lines.
A Summary of the proposed budget for this amendment is as follows:
Additional Design/Plan Fee $11,029.06
Additional Design Survey Fee $ 7,500.00
TOTAL PROPOSED AMENDMENT No. 3 $18,529.06
(Water Cost=$0.00; Sewer Cost=$18,529-06)
The additional design/plan fee was based upon the T.S.P.E. Curve%X 85%X Estimated Construction
Cost(additional water lines only) Utilizing Curve B'= (6.0%X 85%X$216,256)= $11,029.06
Dal-Tech Engineering,Inc. (M/WBE Firm)will continue services for additional survey work
on this project. The additional survey work comprises 40.5% of the overall amendment price.

Dear Mr. Goodman:
DAL-TECH Engineering, Inc. is very pleased to submit a fee proposal to perform additional
sun,eyftig services for the above referenced project in Fort Worth, Texas, As requested in yoiu faxed
letter dated March S.2004 we will perform a design surTey of the following areas,
A. The alley located between Mod]-in Street, West 7u' Street, Virginia Place, and Belle
Place and the associated sanitary sewer line "L-1101" located under said alley as
shown on the attached trap. We will also ineasure the depths of any associated
manholes if we are able to open them.
Fee $1,500m
B. The area along sanitary sewer line 'NI-146" between manlioles "N414 61+73" and
"MH 65-03" as shown or)the attached map. We will also measure the depths of any
associated man-doles if we are able to open them and will lie building comers and any
locatable cleanouts on Lots 3 through 7 of Block 9 on the south side of 4'"Street,
Fee $2,000.00
C. The area within the right-of-way of Dorothy Lane located between sanitary sewer line
11-2129" to the south and"L-212811 to the,north as shown on the attached map. We
will also measure the depths of any associated manholes if we are able to open them.
Fee $1,000-00
D� Lots 6, 7, 9, 9, and 10 of Block 12 enclosed by West View Avenue, West Fourth
Street, and Virginia Street. We will only survey the building corners, property
comers, axed visible sanitary sewer clean-outs, and any other visible evidence of
saritary sewer service lines located within these private properties. A full
topographic s-zvey lucid be pwfbmed for the area between the centerline of West
View Avenue, West Fourth Street, and the right-of-way lines of said streets commoTt
with the boundaries of said Block 12.

This survey will also show all topographic features and grade breaks located within the above-
described areas. An electronic AutoCAD draNving will be provided containing contours at 1-foot
intervals. A ,dat, tin, and ASCII points file will also be provided along with the survey. This proposal
includes the efforts required to gain rlght-of-entry to private properties included in or adjoining the above-
mendoned areas to be surveyed.
DAL-TECH recommends, a lump stun budget of$7,500.00 to complete these surveying services,
If you are in agreement with this 5ce proposal, please sign in the space provided below and return this
acknowledgement to our office. We will deliver the survey within 20 working days of your authorization
to proceed. This will servo as our authorization to proceed and our right to enter the property for
surveying purposes.
