I (No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and Camp, Dresser & McKee, Inc. , (Engineer) made and entered into City Secretary Contract No. 27761, (the Contract) which was authorized by the City Council by M&C C-19103 on the 28th day of May 2002, and subsequently revised by one previous amendment; and WHEREAS, the Contract involves engineering services for the following project: Engineering contract for the design of Northside II 48-inch Water Main Extension Contract 2, Sections A and B. WHEREAS, it has become necessary to execute Amendment No. 2 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated September 14, 2004, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $13, 040 .00 . 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $1,322, 957 .52 . 3 . All other provisions of the Contract which are not expressly amended herein shall remain in full force and effect. EXECUTED on this the day of 2004, in Fort Worth, Tarrant County, Texas. City of Fort Worth Department of Engineering 1000 Throckmorton Avenue Fort Worth,Texas 76102 Subject: Holly/Northside II Water Main Extension,Contract 2,Sections A&B,from Northside Pump Station to Harmon Road,Engineering Amendment Two Dear Mr.Sholola: The City of Fort Worth has requested additional engineering services of CDM on the subject project. CDM's engineering services contract for this project was executed on May 28,2002, under City Secretary Contract Number 27761. The lump sum amount of the contract is currently$1,309,917.52, including Amendment number 1 of$44,586.22,approved on August 14,2003. The design contracts included 64,100 linear feet of 54-inch and 48-inch waterline, associated waterline appurtenances,miscellaneous water and sewer line replacements, and railroad/highway crossing. The waterline will transport water from the Northside Pump Station to Heritage Trace Parkway. We are pleased to provide these services to you,and have provided a listing of the out of scope items and the associated fees on the attached tables. The total for this amendment number 2 is$13,040.00 resulting in a revised total contact amount of$1,322,957.52. If you need additional information or have questions please advise.
