Adoption Event Application
PetSmart Charities and PetSmart stores have formed a partnership to help promote multi-agency adoption events at
PetSmart stores. This is an application for approval to HOST an event at PetSmart. It is possible that an event will be
approved but full or partial sponsorship funding may be unavailable.
PART 1: Store Manager completes this page and submits no later than eight weeks prior to event (unless
otherwise communicated via CASE).
Store Manager must complete and submit Event Request on Fetch/Adoptions to initiate landlord approval

STORE MANAGER RESPONSIBILITIES CHECK LIST:
❑ Evaluate and confirm that the selected space is capable of handling this size event(will not impede foot traffic,vehicular traffic,
parking,etc.). Outdoor/Parking Lot Events may occur on premises outside of a store(sidewalk,grassy area or actual
parking lot) dependent on landlord approval.
❑ Verify the lead organization has obtained any required local licenses or permits(i.e. City, Fire Marshal). Permit costs for must
be included in the lead partners budget for reimbursement. Date verified:
❑ During the event, provide a PetSmart space to include Salon,Training, PetsHotel, and/or Banfield if applicable at your store.

Participant-Required Criteria:
6 Partner will support the promotion of the event via approved marketing such as websites, media sponsorship,
social media, signage, flyers, with mention of PetSmart Charities in press release and any publication with the
approved PetSmart Charities logo
G At Ieastl00 percent of adoptions must be initiated on-site
6 Participant must ensure that enough adoptable pets are available to meet or exceed goal (best practice is to have
double the amount of pets available to achieve goal)
6 There must be no competing vendor services, products, fundraisers (without PetSmart Charities approval)
6 All pets at the event will be spayed or neutered
6 Partner must provide secure kennel runs or cages/crates and a separate area for cats
6 Partner must provide perimeter fencing
6 The event must have tents with panels to shield from sun, rain and inclement weather
6 Temperatures must range between 600 and 90° (15-32.2°C) in the areal in which adoptions are taking place to be
considered safe. If temperatures are not within this range, adoptions cannot take place outside.
6 There must be a secure, separate meet-and-greet area(s) (for dogs and cats)

BUDGET: Must be completed by Lead Partner and signed by Executive Director/President or authorized
representative - Return to yourstore manageralong with all required attachments.
PetSmart Charities sponsorships for adoption events are limited, as we must spread the funding across many
communities. We cannot cover costs for vehicles, gas, staffing, t-shirts, food and beverage, or other expenses not directly
connected to adoptions at the event. Once approved, funding for the approved, budgeted items will be issued to
the lead partner, who will be responsible for submitting an expense report thirty (30)days after the event. The report
must include all invoices and cancelled checks evidencing the expenditures of the Grant. Any changes to the budget must
be pre-approved by sending an email to adoptions(o-)ssg.petsmart.com

➢ To reduce expenses for rented equipment, try these ideas:
■ If you will be using these items more than once a year, it may be less expensive to buy rather than continue to rent.
■ Contact schools, churches, temples, YMCA/YWCA, and other community buildings about borrowing these items or
renting at a much lower fee than a "party service" will charge.
■ Collaborate with other local agencies (animal and non-animal interests). Work together and buy these items to be
shared during events. If each agency uses them once annually, you'll all save on rental costs. Use the same idea
to collect crates.

As part of the event we've been invited to participate in, our organization agrees to:
1. If event is at a PetSmart store location ensure all in-store adopters complete and sign the PetSmart°Adoption
Release Form.
2. If event is at a PetSmart store location, ensure staff or volunteers take adopters into the store to scan PetSmart
Adoption Release Forms before the adopter leaves. Scanning validates the adoption and determines accurate
Adoption Rewards payments.
3. Provide a clean, safe environment for healthy, friendly, adoptable pets.
4. If event is at a PetSmart store location, refrain from selling of products and/or fundraising.
5. Provide total adoption numbers after the event to PCI, broken out by species (dogs, cats, small pets) and
participating organization.
6. If event is at a PetSmart store location, only approved PetSmart Charities partners can participate.
7. If the event is at an approved off-site venue the lead organization will receive a form for tracking adoptions that
each participating organization must complete.
As part of the event we've been invited to participate in, our organization agrees to:
6 Greet and get to know pet parents—Your staff and volunteers love pets and so do our pet parents and
associates. This is easy!
G Treat everyone as potential adoptive pet parents —"I'mjust looking" sometimes means "I'm shy." They may
"just be looking" for someone else to refer to you!
6 Determine their needs—As you get to know them, you'll discover their image of the perfect pet.
Ce Educate, inform, and provide alternatives—You have the expertise that a potential adopter needs! Educate
them on the adoption process, inform them of the responsibility of having a pet as well as their desired pet's
history, provide alternatives if an adoption is not in the pet's and their best interest.

I, the undersigned person, on behalf of Applicant, represent to PetSmart Charities, Inc. ("PCI") that: (1) 1 am authorized to
submit this application and make all representations provided in and with this application; and (2) to the best of my
knowledge, the information contained in this application is true and accurate. On behalf of the Applicant and its officers,
directors, employees and volunteers, I grant permission to PCI, its staff and any other designated representatives to make
inquiries with, and request documentation from, third parties about Applicant and the contents of this application, to assist
in the evaluation of this grant request.
In addition, as of the date of signing, the Applicant represents to PCI that (1) the Applicant is either(a)an organization
exempt from federal income tax under Section 501(c)(3)of the Internal Revenue Code of 1986 (the "Code") which is not a
private foundation under Section 509(a) of the Code, (b) a governmental entity identified in Section 170(c)(1) of the Code
that will use the grant for exclusively public purposes, or(c)an"Indian tribal government," as described under Section
7701(a)(40)of the Code, that is treated as a State under Section 7871 of the Code and that will use the grant for
exclusively public purposes; (2)the Applicant validly bolds and maintains all licenses, permits, and registrations, and has
satisfied all similar requirements, necessary for its lawful operation; (3) the Applicant is in compliance with all applicable
local, state, tribal and federal laws, regulations and other requirements to which the Applicant is subject; and (4) the
Applicant is not on any federal terrorism"watch list"and all Grant Funds will be used in compliance with all applicable anti-
terrorist financing and asset control laws, statues and executive orders. The Applicant agrees to notify PCI promptly in
writing of any change in the information represented herein.
