(No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and Dannenbaum Engineering Corporation, (Engineer) made and entered into City Secretary Contract No. 24419, (the Contract) which was authorized by the City Council by M&C 17125 on the 17th day of November 1998; and subsequently revised by two previous amendments; and WHEREAS, the Contract involves engineering services for the following project: Engineering contract for the design of pavement reconstruction and water and sanitary sewer line replacement on Sanborn Avenue (Scott to East Lancaster) ; Camilla Street (Conner to South Beach) ; Koch Street (Conner to South Beach) ; Parkdale Street (Koch to North Dead End) and Sargent Avenue (East Lancaster to Meadowbrook) . WHEREAS, it has become necessary to execute Amendment No. 3 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated May 11, 2000, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $1,718.17 (Water Department; sewer cost) . 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $142,262 . 95 (TPW; $77, 970. 49, Water; $31,297.88, Sewer; $32,994 . 63) . uf�(�BOG�� �CuG?D 3. Nos. 2178 through 2182, Design Fee Amendment No. 3 Dear Ms. Johnson, This letter is to provide information regarding the change in scope on the Water Department work on the above referenced project. The water department has requested that Dannenbaum Engineering add the design and construction of sanitary sewer lateral L-1661 from existing Station 2+37 to Station 4+80 to our contract. For your use, the Supplemental Fee Summary and the Engineer's Opinion of Construction Cost are attached. Our opinion of construction cost is $18,507.00 with additional surveying and engineering costs of$1,718.17. Since the design is the replacement of an existing lateral in place, the necessity of utility easements are not anticipated and such costs are not included. Douglas Rademaker, Director, DOE and Mike Groomer, Assistant City Manager FROM: Dena Johnson, DOE DATE: June 6, 2000 RE: AMENDMENT NO. 3 TO PAVEMENT RECONSTRUCTION AND WATER AND SANITARY SEWER REPLACEMENTS ON SANBORN AVENUE (SCOTT TO EAST LANCASTER); CAMILLA STREET (CONNER TO SOUTH BEACH); KOCH STREET (CONNER TO SOUTH BEACH); PARKDALE STREET (KOCH TO NORTH DEAD END) AND SARGENT AVENUE (EAST LANCASTER TO MEADOWBROOK). SEWER PROJECT NO. PS46-070460134010 This amendment will reimburse Dannenbaum Engineering Corporation for the additional engi- neering services they provided on the Water Department portion of the design contract. It has been determined that it is necessary to replace a severely deteriorated 250 feet section of sewer lateral L-1661 which is located within close proximity to this project. The poor condition of the identified lateral was unknown prior to award of contract. Dannenbaum Engineering Corporation, has requested reimbursement in the amount of$1,718.17 (Water Department cost) for the additional services that were provided thereby increasing the con- tract fee from $140,544.78 (TPW; $77,970.44, Water; $31,297.88, Sewer; $31,276.46) to $142,262.95 (TPW; $77,970.44, Water; $31,297.88, Sewer; $32,994.63). I believe this fee is fair and reasonable for the additional services that were provided.
