D " AMENDMENT No.2 TO CITY SECRETARY CONTRACT WHEREAIS, the City of Fort Worth (CITY) and Dunaway Associates, L.P. , (ENGINEER) made and entered into City Secretary Contract No. 46480, (the CONTRACT) which was authorized by M&C C-27191 on the 17th day of February, 2015 in the amount of $351,569.00; and WHEREAS, the contract was subsequently revised by: Amendment No. 1 in the amount of $33, 753 .00 which was administratively authorized on April 24, 2015; and WHEREAS, the CONTRACT involves engineering services for the following project: Water Main and Sanitary Sewer Replacement contract 2015, WSM-B; and WHEREAS, it has become necessary to execute Amendment No. 2 to the CONTRACT to include an increased scope of work and revised fee. NOWTHEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated May 14, 2015, a copy of which is attached hereto and incorporated herein. The cost to the City for the additional design services to be performed by Engineer totals $38,810 .00. 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services :shall be rrn an amount of $424,132 .00 . Project Manager City of Fort Worth Water Department 311 W 10"' Fort Worth, Texas 76102 Reference: Amendment No. 2 Water and Sanitary Sewer Replacement Contract 2015, WSM-B City Project No. 02436 Dear Mr. Norwood: Dunaway Associates, L.P. is pleased to present this Amendment No. 2 for the following additional services that are beyond our original scope of services. Dunaway proposes to provide the additional design plans for 500 LF of 8-inch water main along Mulkey St., Colvin Ave, Cantey St., and Grove Street, including preparation of TxDOT Utility Permit for each location. It is understood that the City will submit the permit to TxDOT and coordinate with TxDOT. It is also understood that detailed Traffic Control Plans will not be required at each location by TxDOT, and that standard details will be utilized for the permit package. Survey and Design services for additional 160 LF of 16-inch Water Main plan and profile along Colvin Ave extending east across Evans Ave into the Morningside Elementary School property. The intent is to replace all remaining existing 16-inch Cast Iron water main dating back to year 1912. It is assumed that all work will be completed within existing permanent waterline easement on School property, and the City will handle all coordination with the School representatives for the work on their property. In addition, it is assumed that a detailed traffic control plan will not be required for crossing Evans Ave, and standard traffic control plan and details shall be utilized. In addition, Dunaway will provide Survey tasks for the Valley View Drive Sanitary Sewer Main L- 3792 replacement per sub-consultant proposal Items 1, 2, and 3 attached. The Survey will be used to complete the 30% design. The contractor hired by the City will perform the d-holes and Dunaway's survey will tie down the holes in the field. Dunaway will also prepare a Union Pacific Railroad (UPRR) Right of Entry Application for anticipated construction work within Grove St. adjacent to the railroad right of way. FEE Dunaway proposes to provide the above described scope of work for a LUMP SUM fee of $38,810.00. Only those services specifically mentioned in the Scope of Work section are offered as a part of this proposal.
