TO CITY SECRETARY CONTRACT
EREAS, the City of Fort Worth (CITY) and EJES,
Incorporated, (ENGINEER) made and entered into City Secretary
Contract No. 43806, the CONTRACT) which was authorized by M&C
C-25848 on the 18th day of September, 2012 in the amount of
$1931110 . 00; and
EREAS, the contract was subsequently revise by:
Amendment Number 1 in the amount of $119,387 . 00 which was
authorized by M&C C-26877 on the day of July 22, 2014; and
EREAS, the CONTRACT involves engineering services for
the following project:
Sanitary Sewer Rehabilitation Contract 91; and
EREAS, it has become necessary to execute Amendment
No. 2 to the CONTRACT to include an increased scope of work
and revised fee.
NOWTREREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:

Article I of the CONTRACT is amended to include the
additional engineering services specified in a proposal dated
September 25th, 2015, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional
design services to be performed by Engineer totals $48, 942 . 00 .
2 .
Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be
an amount of $361,439 . 00 .
3 .
All other provisions of the Contract, which are not
expressly amended herein, shall remain in full force and

Additional Design, Research and Survey Work
Dear Mr. Chaubey:
Per your request we are pleased to submit this Amendment No. 2 for additional design services required to
complete the above referenced project. The requested modifications will be outside of our scope of services
as set forth in City Secretary Contract No. 43806, therefore this Amendment No. 2 is being developed to
further identify these services and summarize the associated costs. The additional services required to
prepare final construction documents will include the following:
® Design Additional 1,182 LF of 8"Waterline in Anita Avenue, 1,018 LF of 8"Waterline in Winfield Avenue,
1,567 LF of 8"Waterline in Cherokee Trail, and 1,055 LF of 8"Waterline in Seminary Drive and Survey of the
location and size of the existing water meters along those streets:
1. Anita Avenue (Manderly Pl. to South Dr.)
2. Winfield Avenue (Anita Ave. to Carolyn Rd.)
3. Cherokee Trail(West Seminary Dr. to Suffolk Dr.)
4. Seminary Drive (Trail Lake Dr. to South Dr.)
• Research and Verify locations of water lines and any conflicts with
Storm Drains and Sanitary Sewer Lines
• Revise Summary of Quantities and Cost Estimate
• Obtain surveyed location of existing water meters and sizes of meters(4,822 LF)
We are requesting your assistance in the execution of this Amendment No. 2 in order for us to expeditiously
perform these services.
The attached Level of Effort LOE) of spreadsheet , outlines the additional tasks, personnel and man-hours
we anticipate to be required to make the requested revisions to the construction documents. The total fee
for these additional services will be an amount not to exceed $48,942.
In order to avoid any further delay in the project schedule, EJES, Inc. will proceed to make the requested
revisions outlined above at this time.

These Amendment No. 2 services will be performed with the good faith understanding that an executed
agreement for additional compensation is forthcoming and will be received prior to submittal of the final
documents. The total contract amount to date, including amendments, is as follows:
Original Contract Amount = $193,110.00
Amendment No. 1 = $119,387.00
Amendment No. 2 = $ 48,942. 00
New Contract Amount = $361,439.00
The City MWBE goal for this contract is 22%. The total amount to be paid to Arredondo, Zepeda, & Brunz,
LLC (MWBE) for topographic survey from the total Amendment No. 2 of$48,942 is $5,995 which is 12.25%.
The total amount to be paid to M,NVBE on total contract amount of$361,439 is $119,762, which is
32.13%; therefore meeting the City MWBE goal of 22%.
