HomeMy WebLinkAboutOrdinance 15007,~ Ordinance No AN ORDINANCE INCREASING THE ESTIMATED RECEIPTS AND APPROPRIATIONS IN THE GRANTS FUND IN THE AMOUNT OF $10,000 FOR THE PURPOSE OF FUNDING AUTHORIZATION TO EXECUTE AMENDMENT NO 1 TO THE CITY SECRETARY CONTRACT N0.27029 WITH THE TEXAS DEPARTMENT OF HEALTH AND THE TEXAS DIABETES PROGRAM, MAKING THIS ORDINANCE CUMULATNE OF PRIOR ORDINANCES AND REPEALING ALL PRIOR ORDINANCES IN CONFLICT HEREWITH, AND PROVIDING AN EFFECTNE DATE. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH, TEXAS. SECTION 1 That in addition to those amounts allocated to the various City departments for Fiscal Year 2001-02 and in the Budget of the City Manager, there shall also be increased estimated receipts and appropriations in the Grants Fund in the amount of $10,000 for the purpose of funding authorization to execute Amendment No. 1 to the City Secretary Contract No. 27029 with the Texas Department of Health and the Texas Diabetes Program. SECTION 2. That should any portion, section or part of a section of this ordinance be declared invalid, inoperative or void for any reason by a court of competent jurisdiction, such decision, opinion or judgment shall in no way impair the remaining portions, sections, or parts of sections of this ordinance, which said remaining provisions shall be and remain in full force and effect. SECTION 3 That this ordinance shall be cumulative of Ordinance No. 14745 and all other ordinances and appropriations amending the same except in those instances where the provisions of this ordinance are in direct conflict with such other ordinances and appropriations, in which instance said conflicting provisions of said prior ordinances and appropriations are hereby expressly repealed. SECTION 4 This ordinance shall take effect upon adoption. APPROVED AS TO E - Ass' a City Atto e ADOPTED AND EFFECTIVE. `~'' ~ Q City of Fort Worth, Texas Mayor and Council Communication DATE REFERENCE NUMBER LOG NAME PAGE 3/5/02 **C-18985 50DIABETES 1 of 2 SUBJECT ADOPTION OF APPROPRIATION ORDINANCE AND AUTHORIZATION TO EXECUTE AMENDMENT NO 1 TO CITY SECRETARY CONTRACT NO 27029 WITH THE TEXAS DEPARTMENT OF HEALTH AND THE TEXAS DIABETES PROGRAM/COUNCIL FOR A DIABETES AWARENESS AND EDUCATION IN THE COMMUNITY GRANT FOR A FOURTH YEAR FROM SEPTEMBER 1, 2001, TO AUGUST 31, 2002 RECOMMENDATION It is recommended that the City Council 1 Authorize the City Manager to execute Amendment No 1 to City Secretary Contract No 27029 with the Texas Department of Health (TDH) and the Texas Diabetes Program/Council (TDP/C) for $10,000 to be used to supplement the Chronic Disease Self-Management Program within ,the Diabetes Awareness and Education in the Community Grant for the fourth year from September 1, 2001, to August 31, 2002, and 2 Adopt the attached appropriation ordinance increasing estimated receipts and appropriations in the Grants Fund by $10,000 DISCUSSION On August 21, 2001 (M&C C-18716, Ordinance No 14757), the City Council authorized the continuation grant contract with the TDH and the TDP/C The main objective of this Diabetes Awareness and Education in the Community (DAEC) continuation grant is to continue the development and implementation of a community project that reduces the burden of diabetes, increases the understanding of diabetes, and builds community and organizational capacity which is focused and prevention-oriented The DAEC is modeled after the Healthy Communities 2000 Model Standards, the Center for Disease Control and Prevention Planned Approach to Community Health and Diabetes today The Fort Worth Public Health .Department plans to accomplish this objective by the following Forming regional coalitions in the City with organizations that are interested in increasing awareness of diabetes prevention and maintenance, including a report structure and mechanism, and • Design and implementation of an awareness campaign that will include media exposure and grass roots level work; and • Utilizing existing resources through TDH to evaluate the program The supplemental $10,000 will be used to enhance the Chronic Disease Self-Management (CDSM City of Fort Worth, Texas Mayor and Council Communication DATE REFERENCE NUMBER LOG NAME PAGE 3/5/02 **C-18985 50DIABETES 2 of 2 SUBJECT ADOPTION OF APPROPRIATION ORDINANCE AND AUTHORIZATION TO EXECUTE AMENDMENT NO 1 TO CITY SECRETARY CONTRACT NO 27029 WITH THE TEXAS DEPARTMENT OF HEALTH AND THE TEXAS DIABETES PROGRAM/COUNCIL FOR A DIABETES AWARENESS AND EDUCATION IN THE COMMUNITY GRANT FOR A FOURTH YEAR FROM SEPTEMBER 1, 2001, TO AUGUST 31, 2002 Program This is a program designed by Stanford University It is classroom based and is designed to empower people to self-manage their chronic diseases. The program is six weeks long, 2.5 hours per week. Although the CDSM Program is not disease specific, participants will gain the skills necessary to deal with the emotional, medical, and physical aspects of their disease Stanford University has shown that through this program, people with chronic diseases have fewer incidences of hospitalization and report better levels of health The funding will allow the Fort Worth Public Health Department to reach more people and allow a greater number of citizens to participate in the CDSM Program This will be achieved with incentives and a campaign to educate doctors about the program so that they will refer their patients to it.
