Program This is a program designed by Stanford University It is classroom based and is designed to
empower people to self-manage their chronic diseases. The program is six weeks long, 2.5 hours per week.
Although the CDSM Program is not disease specific, participants will gain the skills necessary to deal
with the emotional, medical, and physical aspects of their disease Stanford University has shown that
through this program, people with chronic diseases have fewer incidences of hospitalization and report
better levels of health
The funding will allow the Fort Worth Public Health Department to reach more people and allow a
greater number of citizens to participate in the CDSM Program This will be achieved with incentives
and a campaign to educate doctors about the program so that they will refer their patients to it. This will
provide a captive audience that has a great need for the CDSM Program
