AN ORDINANCE INCREASING ESTIMATED RECEIPTS AND APPROPRIATIONS
IN THE GRANTS FUND IN THE AMOUNT OF $100,000 FROM AVAILABLE
FUNDS FOR THE PURPOSE OF FUNDING A CONTINUATION OF A GRANT
CONTRACT WITH THE TEXAS DEPARTMENT OF HEALTH AND THE TEXAS
DIABETES PROGRAM/COUNCIL FOR DIABETES AWARENESS AND
EDUCATION IN THE COMMUNITY, MAKING THIS ORDINANCE CUMULATIVE
OF PRIOR ORDINANCES IN CONFLICT HEREWITH, AND PROVIDING AN
EFFECTIVE DATE.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH,
TEXAS.
SECTION 1
That in addition to those amounts allocated to the various City departments for Fiscal Year 2001-02 in the
Budget of the City Manager, there shall also be increased estimated receipts and appropriations in the Grants
Fund in the amount of $100,000 for the purpose funding a continuation of a grant contract with the Texas
Department of Health and the Texas Diabetes Program/Council for diabetes awareness and education in the
community
SECTION 2.
That should any portion, section or part of a section of this ordinance be declared invalid, inoperative or void
for any reason by a court of competent jurisdiction, such decision, opinion or judgment shall in no way impair
the remaining portions, sections, or parts of sections of this ordinance, which said remaining provisions shall
be and remain in full force and effect.
SECTION 3
That this ordinance shall be cumulative of Ordinance No. 14745 and all other ordinances and appropriations
amending the same except in those instances where the provisions of this ordinance are in direct conflict with
such other ordinances and appropriations, in which instance said conflicting provisions of said prior
ordinances and appropriations are hereby expressly repealed.
SECTION 4
This ordinance shall take effect upon adoption.

FOR THE FIFTH YEAR WITH THE TEXAS DEPARTMENT OF HEALTH AND THE
TEXAS DIABETES PROGRAM/COUNCIL FOR DIABETES AWARENESS AND
EDUCATION IN THE COMMUNITY
RECOMMENDATION
It is recommended that the City Council
Authorize the City Manager to accept and execute a continuation to a grant contract with the Texas
Department of Health (TDH) and Texas Diabetes Program/Council (TDP/C) in the amount of
$100,000 to be used to increase diabetes awareness and education at the community level from
September 1, 2002, to August 31, 2003, and
2 Adopt the attached appropriation ordinance increasing estimated receipts and appropriations by
$100,000 in the Grants Fund, and
3 Authorize anon-interest bearing interfund loan from the General Fund to the Grants Fund in the
amount of $25,000 for interim financing for this grant pending contract execution to ensure a smooth
transition from one grant funding period to another
DISCUSSION
Diabetes is an epidemic and a public health crisis in Texas The incidence rate has increased tenfold in
the last 60 years (3 7 per 1,000 in 1936, to 30 6 per 1,000 in 1995) The estimated prevalence of diabetes
in Texas is 6 5% Diabetes impacts minorities disproportionately, i e , 11 4% for African Americans,
and 9 3% for Hispanic Americans Based on the cost methodology developed by the University of
Texas ("Direct and Indirect Costs of Diabetes in Texas", 1992, by Dr David Warner, LBJ School of
Public Affairs, et al ), it is estimated that the annual cost of diabetes in Texas is approximately $4 billion
Using this same cost methodology, the financial burden of diabetes annually for the City of Fort Worth
is estimated to be $102,168,000 Yet, we continue to spend health care dollars on downstream
medical technology and treatment, rather than focusing on prevention and intervention
Type 2 Diabetes may be preventable through proper nutrition and physical activity It is estimated that
50% of the blindness and 60% of the amputations are preventable Even with new medications and
management me#hodology, diabetes is still the leading cause of blindness, kidney disease, and
.amputations in Texas, making it one of our most serious, costly, and common public health threats
The main objectives of this Diabetes Awareness and Education in the Community (DAEC) continuation
grant is to continue the development and implementation of a community project that reduces the
burden of diabetes, increases the understanding of diabetes, and builds community and organizational
capacity It is community focused and prevention oriented It has achieved many objectives in the past
four years

FOR THE FIFTH YEAR WITH THE TEXAS DEPARTMENT OF HEALTH AND THE
TEXAS DIABETES PROGRAM/COUNCIL FOR DIABETES AWARENESS AND
EDUCATION IN THE COMMUNITY
The program conducted an initial telephone survey to gauge the level of knowledge and awareness in
the community In this, the final year of the grant, another survey will be conducted to determine the
effectiveness of the program in raising the level of knowledge and awareness in the community A few
of the activities accomplished by the program in the years following the survey have been
a) 84 community and corporate health fairs reaching. 42,000 citizens, and
b) 117 educational presentations in the community reaching 1,385 citizens, and
c) Bus bench and radio campaigns as follows
• A total of 75 bus benches, 360 interior bus placards, and 15 bus tails with over
50 million exposures in three years, and
• Over 120 targeted radio spots and 80 public service announcements with over
1 million exposures, and
d) 119 restaurant surveys to gauge healthy menus, and
e) Partnerships with the University of North Texas Health Science Center, the Tarrant
County Health Department, the Tarrant County Diabetes Collaboration, Inc., the
Fort Worth Independent School District, the City of Fort Worth Wellness Program,
the Urban Inter-Tribal Center of Texas, the National Council of La Raza, and
others, and
f) The Chronic Disease Self Management Program (CDSM)
The CDSM program was designed by Stanford University It is a classroom-based six week program
designed to empower people to self manage their chronic diseases. Although the program is not
disease specific, participants will gain the skills necessary to deal with the emotional, medical, and
physical aspects of their disease Stanford University has shown that through this program, people with
chronic diseases have fewer incidences of hospitalization and report better levels of health In two
years, 157 people have gone through the program
The Public Health Department plans to continue these strategies in the next year, and in addition will
coordinate referral systems through a variety of organizations, and work with elementary schools to
facilitate healthier meal options
