AN ORDINANCE INCREASING THE ESTIMATED RECEII'TS AND APPROPRIATIONS IN THE
GRANTS FUND IN THE AMOUNT OF $100,000 00 CONTINGENT ON THE RECEIPT OF THE
GRANT AWARD FROM THE DEPARTMENT OF HEALTH AND HUMAN SERVICES OFFICE OF
THE CENTERS FOR DISEASE CONTROL AND PREVENTION FOR THE PURPOSE OF FUNDING A
CONTRACT WITH COOK CHILDREN' S COMMUNITY CLINICS FOR ASTHMA EDUCATION AND
SERVICES, PROVIDING FOR A SEVERABILITY CLAUSE, MAKING THIS ORDINANCE
CUMULATIVE OF PRIOR ORDINANCES AND REPEALING ALL PRIOR ORDINANCES IN
CONFLICT HEREWITH, AND PROVIDING AN EFFECTNE DATE.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH, TEXAS.
SECTION 1
That in addition to those amounts allocated to the various City departments for the Fiscal Year 1998-99 and m the Budget of
the City Manager, there shall also be increased estimated receipts and appropriations in the Grants Fund in the amount of
$100,000 00 contingent on the receipt of the grant award from the Department of Health and Human Services Office of the
Centers for Disease Control and Prevention for the purpose of funding a contract with Cook Children's Community Clinics for
asthma education and services.
SECTION 2.
Should any portion, section or part of a section of this ordinance be declared invalid, inoperative or void for any reason by a
court of competent jurisdiction, such decision, opinion or judgment shall m no way impair the remaining portions, secfions, or
parts of sections of this ordinance, which said remaining provisions shall be and remain in full force and effect.
SECTION 3
That this ordinance shall be cumulative of Ordinance No: 13111 and all other ordinances and appropriations amending the
same except in those instances where the provisions of this ordinance are in direct conflict with such other ordinances and
appropriations, in which instance said conflicting provisions of said prior ordinances and appropriations are hereby expressly
repealed.
SECTION 4
This ordinance shall take effect and be in full force and effect and after the date of its passage, and it is so ordained.

Subject APPROPRIATION ORDINANCE AND CONTRACT WITH THE DEPARTMENT OF
HEALTH AND HUMAN SERVICES CENTERS FOR DISEASE CONTROL AND
PREVENTION (CDC) FOR A COOPERATIVE AGREEMENT FOR ASTHMA EDUCATION
FOR PARENTS OF ASTHMATIC CHILDREN ENROLLED IN WELFARE-TO-WORK
PROGRAMS
RECOMMENDATION
It is recommended that the City Council
1 Authorize the City Manager to apply for and, if offered, accept and execute a grant contract with the
Department of Health and Human Services Office of the Centers for Disease Control and
Prevention (CDC) in the amount of $100,000, and
2. Adopt the attached appropriation ordinance increasing appropriations and estimated receipts by
$100,000 in the Grants Fund, and
3 Contract with the Cook Children's Community Clinics for asthma education and services (contingent
on receipt of the grant award), and
4 Apply indirect costs estimated to be $1,500 based on an indirect cost rate of 22.93% of salary and
benefits, which is the most current rate for this department.
DISCUSSION
The Centers for Disease Control and Prevention (CDC) announced funding fora cooperative
agreement for the development,- implementation, and evaluation of an asthma education and
management program for families enrolled in Welfare-to-Work programs. The purpose of the grant is to
bring together public health expertise and welfare reform to provide the asthma education and
management techniques necessary to enable families enrolled in the Welfare-to-Work program to finish
job training and enter the work force
Asthma is a chronic, controllable illness. Controlling asthma symptoms depends on adhering to a daily
regimen of maintenance medication and avoidance of triggers which cause airway inflammation
Urban, low-income children in North Texas are typically exposed to high levels of indoor allergens such
as dust mites, cat, dog, and cockroach allergens as well as outdoor allergens such as ragweed, oak
Bermuda grass, and mountain cedar An effective asthma management program will assess the
patient's environment and assist families in planning control measures.
The City of Fort Worth Health Department staff will partner with the Tarrant County Workforce
Development Board in identifying families enrolled in the Welfare-to-Work program who have asthmatic
children The collaboration then broadens to include Cook Children's Community Clinics which will
implement their asthma education and treatment management program, CHAMP (Children's Asthma
Management Program) Additionally, Americaid® Community Care, the pediatric managed care vendor

for Medicaid in Tarrant County, will provide data to be used in the evaluation of the program by staff
from the Health Department's assessment section and from the Texas Department of Health's Bureau
of Epidemiology
Costs incurred in implementing the grant will involve Public Health Department staff coordinating the
project by reporting to CDC, conducting initial environmental and asthma assessments, analyzing all
pertinent data from assessments and study findings, and contracting with the Cook Children's
Community Clinics for asthma education and. services Additional costs may include any supplies
needed to run the program during the year, including some minor equipment purchases. The funding
period would begin September 30, 1998, and end August 1, 1999
FISCAL INFORMATION/CERTIFICATION
The Finance Director certifies that upon acceptance and execution of this grant contract and adoption
of the attached appropriation ordinance, funds will be available, as appropriated, in the Grants Fund
