Ordinance No.
AN ORDINANCE INCREASING THE ESTIMATED RECEIPTS AND APPROPRIATIONS IN THE
GRANTS FUND IN THE AMOUNT OF $10,000 00 SUBJECT TO THE RECEIPT OF THE GRANT
FROM THE TEXAS DEPARTMENT OF HEALTH FOR THE PURPOSE OF FUNDING THE
COMivIL1NITY ORIENTED PRIMARY CARE PROGRAM, PROVIDING FOR A SEVERABILITY
CLAUSE, MAKING THIS ORDINANCE CUMULATIVE OF PRIOR ORDINANCES AND
REPEALING ALL PRIOR ORDINANCES IN CONFLICT HEREWITH, AND PROVIDING AN
EFFECTIVE DATE.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH, TEXAS.
SECTION 1
That in addition to those amounts allocated to the various City departments for the Fiscal Year 1997-98 and in the Budget of
the City Manager, there shall also be increased estimated receipts and appropriations the Grants Fund in the amount of
$10,000 00 subject to the receipt of the grant from the Texas Department of Health for the purpose of funding the Community
Oriented Primary Care Program.
SECTION 2.
Should any portion, section or part of a section of this ordinance be declared invalid, inoperative or void for any reason by a
court of competent jurisdiction, such decision, opinion or judgment shall in no way impau the remaining portions, sections, or
parts of sections of this ordinance, which said remaining provisions shall be and remain in full force and effect.
SECTION 3
That this ordinance shall be cumulative of Ordinance No. 13111 and all other ordinances and appropriations amending the
same except in those instances where the provisions of this ordinance are in direct conflict with such other ordinances and
appropriations, in which instance said conflicting provisions of said prior ordinances and appropriations are hereby expressly
repealed.
SECTION 4
This ordinance shall take effect and be in full force and effect and after the date of its passage, and it is so ordained.

PREVENTION AND COMMUNITY ORIENTED PRIMARY CARE PROGRAMS WITH THE
TEXAS DEPARTMENT OF HEALTH
RECOMMENDATION
It is recommended that the City Council
1 Authorize the City Manager to apply for and, if offered, accept and execute two grant contracts with
the Texas Department of Health, one for $40,000 for the Chronic Disease Prevention Program, and
the other for $10,000 for the Community Oriented Primary Care Program, and
2. Adopt the attached appropriation ordinances increasing appropriations and estimated receipts by
$40,000 and $10,000 in the Grant Fund
DISCUSSION.
Beginning October 1, 1997, the City of Fort Worth Public Health Department reorganized to develop a
population based program to improve the health of our citizens With that reorganization, the Health
Information Assessment Outreach (IAO) Division was created To begin the process, the IAO Division
has begun conducting a survey consisting of 3,000 face-to-face interviews to gather the data for
identification of community needs
Nearly sixty percent of the survey will capture data to assess needs and design interventions for
persons diagnosed with and at risk for cardiovascular disease, cancer, and diabetes Interpretation of
this data will provide a blueprint for our program to create a healthy Fort Worth
The cost for developing the survey instrument, conducting 3,000 surveys to gather the data, entering
the data into spreadsheets for statistical analysis, and analyzing the data is budgeted at $122,000 The
grant cycle ends August 31, 1998 As a result, the funds will be applied immediately toward the
completion of this project.
Receipt of these combined grants of $50,000 will offset the General Fund commitment for this project,
resulting in savings in the Health Department's current operating budget.
